摘要:
Patients randomly assigned and treated at two community facilities, a psychiatric day hospital and an outpatient clinic, are evaluated two and 12 months following termination of treatment. An earlier advantage for schizophrenic patients treated at the day hospital is maintained over time, provided aftercare service is utilizied. Judiciously-used outpatient chemotherapy appears less expensive than, and as effective as, day hospital treatment for non-schizophrenic patients.

During 1967, North-Eastern Dispensary, a nonprofit dental clinic in New York City, provided comprehensive dental services for 86.5 per cent of 362 preschool Head Start children. All were eligible for the New York State Medical Assistance Program; all lived in neighborhoods where about 97 per cent of the population was Negro; most traveled about five miles to reach the clinic The mean of per child was 2.2; 98.1 per cent of the carious teeth required treatment, 50.4 per cent of the children were caries-free. The Medical Assistance Program reimbursed the clinic on a fee-far-visit basis. The mean reimbursement per child was $19.38. This cost would have been $34.15 had private dentists provided similar services. Projections of utilization and costs ore made to an estimated 100,000 preschool poverty children in the city. It is suggested that nonprofit dental clinics such as the one described be considered additional delivery systems for dental care.

The critical health manpower problem in the United States is most evident in the field of dental health. Shortages of dentists are projected, due to rising population and income levels. This inadequacy will be accentuated by third-part payments, like Medicaid. The dental manpower situation in the Boston metropolitan area was assessed. It was found that towns that have low socioeconomic levels, compared with towns that have higher socioceonomic levels, had: (a) fewer dentists relative to their populations; (b) fewer specialists; (c) older dentists, less productive in terms of equipment, employment of auxiliary personnel, and hours worked, who will require replacement soon; (d) dentists who less frequently employ auxiliary personnel; and (c) recent histories of losing rather than gaining dentists. While demand for dental care is expected to increase in all areas in the future, low socioceonomic areas with large numbers of Medicaid-eligihle patients will show dramatic spurts in dental demand. These poverty areas need more dentists, but it is unlikely that this need will be met. This situation is largely masked by the fact that the Boston metropolitan area as a whole has an excellent dental manpower situation. Redistribution of dentists within the Boston metropolitan area will be necessary to meet dental demands resulting from Medicaid.

Previous studies explored two sets of relationships separately—one patient characteristics and hospital use and the other between hospital characteristics and hospital use. This leaves the following questions unresolved. Is the revealed relationship betweenpatientcharacteristics and hospital use a reflection of different medical philosophies practiced by individual hospitals whose distinct modes of practices attract patients with distinct characteristics?Looking at the relations from the other side, is the revealed relationship betweenhospitalcharacteristics and hospital use attributable to the fact that patients at hospitals with different characteristics have different sets of expectations and demands as shaped by their socioeconomic characteristics?To seek answers to these questions, various interaction terms between the variables representing patient characteristics and those for hospital characteristics are formed, and their effects on hospital use are examined. The results provide some new insights into the consumption theory of hospital services.

In conjunction with a Patient Origin Study which surveyed all patients discharged from hospitals throughout most of California, a separate feasibility study was conducted in northeastern California to evaluate the effectiveness of utilizing provisional discharge diagnoses recorded by Medical Record Librarians at the time of discharge as an added factor to incorporate into future traffic pattern analyses. This paper reports on a validation study of the diagnosis portion of the study. A 5 per cent sample (156 cases) of the 3,308 patients discharged from the pilot area was selected for chart review. Approximately 10 per cent of the discharge diagnoses showed discrepancies between the original survey results and the validation review, including 3 per cent of moderate or severe differences in interpretation and 1 per cent numerical transposition. On the basis of these figures, it is felt that the use of provisional discharge diagnoses yields useful information of acceptable accuracy.

Physician-nurse teams using common concepts of levels of patient care can make replicable judgments with respect to the care needs and proper placement of statistically random samples of patients seen in various settings. These data are most useful in identifying over-use, under-use, and the absence of various levels of patient care in the community.
