At Crawford Long Hospital of Emory University, a new system of patient care delivery, defined as collaborative case management, was developed as the organization struggled with implementation of the Deming management method. At the onset of the change process, an evaluation system was established to measure and document specific outcomes for patients, families, providers of care, and the organization. This article traces the development and implementation of the collaborative case management program and describes the evaluation system. Findings highlighting improved patient care outcomes, changes in patient and provider perceptions of care, and benefits to the organization are summarized.

Nurses at Beth Israel Hospital have enjoyed a professional model for years. One of the advantages of this model of nursing is that collegial relationships between professional nurses and other members of the health care team are valued and supported by the institution. The articles to follow provide the reader with examples of collaborative relationships and the positive outcomes for patients, their families, and for providers of care. Collaborative relationships enable practitioners to focus on the needs of patients. During the next several years of health care reform, this focus will ensure continued success in providing quality care to our patients.

The following article is written from the experience of its authors with a collaborative research academic appointment. Many advantages and disadvantages were found. The disadvantages included the division of time, responsibilities, loyalties, and energy required by two work settings rather than one. When the challenges are met, however, the rewards are many for the institution, its professional staff, its patients, and the researcher. The joint research appointment brings to the institution, clinicians, students of the researcher, and to the researcher a broadening of the professional practice and experience. Opportunities develop that would have been impossible in other situations. The quality of the teaching may improve and the quality of patient care is enhanced. The profession is advanced. The authors conclude that the additional time and effort necessary to maintain a collaborative research appointment are more than compensated.

Although collaboration is a much sought-after goal among health care professionals, minimal research has clarified the essential communication elements (behaviors) necessary for collaboration. This series of research studies, grounded in the pragmatic perspective, represents a beginning attempt to identify these behaviors. A total of 270 practicing physicians and nurses responded to open-ended questions and/or a survey assessing the communication elements of content; relationship (aggressive, affirming, and collaborative styles); and opportunity to communicate. Findings suggest elements necessary for collaboration include reliable presentation of relevant data, openness to information presented, and adequate time to communicate. Differences were noted between nurses and physicians as to the relative importance of these behaviors.

摘要:
“Synergism” accurately describes the gains that can be made when total quality management (TQM) and shared governance are employed for reciprocal development. This article explores the relationship between TQM and shared governance from a systems perspective. Systems thinking is the fundamental framework that must be learned by nursing managers. An example of this synergistic process is described from the National Institutes of Health nursing department's experience in implementing TQM and shared governance. The idea that structure is fundamental to problems and solutions when implementing change and focusing upon interdependency issues are the systemic competencies nursing managers need to develop in order to become strong nursing leaders.

A transformational partnership model between a consortium of rural community hospitals and a private Christian university reflects a shared vision for R.N. baccalaureate degree completion. The model includes a communitywide system to promote and sustain accessible, high-quality education for the place-bound R.N. who desires to achieve a B.S.N. Stake holders, experiencing the current climate of change in health care, benefit from this innovative service/education leadership model. The collaboration built on a commitment to common goals and shared resources seeks to prepare individuals who apply critical thinking to practice settings along with the continuum of care.

Changes in staffing toward a higher number of R.N.'s are common in a number of countries today. This article aims at describing the experiences of a management-induced change from mixed to all-R.N. staffing. All R.N.s in a surgical ward at a Swedish university hospital were interviewed two months after such replacements were completed. The nurses described a period of chaos and conflicts but also of support for the idea and enthusiasm for the future. A more careful planning of the change process is to be recommended.
