The move to managed care has expanded the role and opportunities for the advanced practice nurse (APN). The focus on building the continuum of care, the movement out of institutional models of service delivery, and the need for well-developed primary health services have created a major role for APNs. The major obligation for APNs today is to ensure their presence in the places where the design and structuring of the future health care system are unfolding.

摘要:
Advanced practice nurses (APNs) of today have incorporated many roles, sucli as educator, administrator, and consultant, into their practices. However, many APNs have not integrated the role of political activist. Kingdon's model of agenda setting serves as a framework to illustrate how the APN can use nursing practice to shape the agenda-setting component of public policy toward improved patient-client well-being. The author provides rationale for redefinition of specialty practice and offers expert advice on where and how to act prior to seeking legislative activity at the state or national level.

Reduced risk of complex response is proposed as 1 category of outcomes sensitive to the effects of advanced practice nursing. Processes of risk assessment and risk management are described. The goals are to identify complex responses whose occurrence is high in human and/or economic costs, to assess factors associated with these complex responses, to provide good information to support interventions that reflect the values of the discipline, and to make the effectiveness of advance practice registered nurses visible.

The recognition of the effects of a failure to attain continuity of care is driving many of the changes in U.S. health care. This article explores the relationship of continuity of care and the broader concept of a continuum of care. Actions that advanced practice nurses, educators, and organizations can take to influence its development are described within the context of three issues: (1) recognition of continuity of care myths, (2) the use of financial mechanisms to promote the construction of a continuum of care, and (3) the development of continuum of care building skills of advanced practice nurses.

With funding from n U.S. Department of Housing and Urban Development contract awarded to a private university, advanced practice nurse faculty established a nurse-managed wellness clinic in an apartment building populated by predominantly African American older people. Ethnographic methods were used to ensure culturally competent care. The clinic provided nursing, pharmacy, and health sciences faculty and students with community-focused clinical experiences as interdisciplinary team members.

This article investigates the effects of multiculturism and diversity on the ability of the preoperative certified registered nurse anesthetist (CRNA) educator to convey effective information to the culturally diverse surgical patient. It looks at the importance of preoperative instruction across all populations in relation to hospital length of stay, need for postoperative pain medication, ambulation, and postoperative urinary retention. The article extrapolates these data to the culturally diverse population and gives a concrete plan of care for the advanced practice CRNA to modify teaching methods to facilitate the teaching-learning encounter in the culturally diverse population.

The role of the professional practice leader is to provide leadership in the transformation of nursing practice from a provider focused model to a patient focused model. Nursing standards for patient focused care were developed to define the quality of the nurse-person relationship in a manner consistent with Parse's theory of human becoming. Multiple strategies have been developed to challenge nurses to redefine the purpose, vision, and core values by which they practice. It is only through uncovering the values and beliefs of every person that nurses will be able to care for individuals whom they recognize as unique human beings.
