摘要:
Although many of the changes in today's health care arena have been beneficial, there continues to be persistent dissatisfaction with the nation's health care delivery system. The provision of outcome-oriented, cost-effective health care is no longer a goal. It is a mandate. To accomplish this mandate, the relationship between the costs, quality and desired outcomes of care, and the processes involved in providing care must be reexamined. This article describes factors stimulating the American Nurses Association to develop nursing-sensitive quality indicators, details development of the National Database for Nursing Quality Indicators (NDNQI), and discusses the implications of nursing-sensitive indicators and NDNQI for nurse administrators. Nursing's quality indicators can be of value in the development of evidence-based health care report cards and can be used to direct change. We must move forward together in claiming the future of nursing through nursing-sensitive quality indicators.

摘要:
Improving physician documentation ensures that the patient's clinical course is clearly recorded. North Carolina Baptist Hospital implemented interventions addressing physician documentation to ensure the assignment of the most appropriate diagnosis-related group (DRG) when it launched the Clinical Documentation Management Program (CDMP). Collaboration between registered nurses trained as clinical documentation consultants (CDC) and certified coding specialists as well as ongoing physician education has resulted in more accurate and complete documentation in the medical record.

摘要:
The nursing shortage and specifically the shortage of new nursing leaders has fueled heightened recognition of the need for leadership development. This article considers the application of the quality improvement process to leadership curriculum development for the Center for Nursing Leadership's (CNL) Journey Toward Mastery program. Both the postmodernist approach and a commitment to scholarship are used as a framework to develop, evaluate, and revise the program. Further, the importance of symmetry between philosophical perspective and curriculum design are illustrated.

摘要:
The authors describe the evolution of a pediatric quality management program in a large community hospital that has rapidly grown in size and complexity to be a tertiary care center. Positive changes in senior leadership expectations, physician involvement, and training in performance improvement methodologies contributed to the development of a more comprehensive program. The authors from several specialty areas of the pediatric service illustrate three quality and performance improvement projects that they used in coping with the challenges of fast growth and higher acuity.

Bill Gates states inBusiness @ The Speed of Thoughtthat “the 2000s will be about velocity.” This “decade of velocity” is driven by the flow of digital information. Health care organizations should develop a “digital nervous system” that is distinguished from a network of computers by the accuracy, immediacy, and richness of the information it brings to health care workers and by the insight and collaboration made possible by this information. This article provides a clinical engineering perspective on technology in health care in an era of rapid change. It discusses how clinical engineers work with health care professionals to manage and adapt to technological, economic, social, and regulatory forces that are redefining our approach to health care delivery.

Bristol Hospital, a 134-bed community health care system in Connecticut, has consistently been nationally ranked in the top tier of hospitals for its patient satisfaction scores. A key to this long-term distinction has been its commitment to a culture built around patient satisfaction and service excellence. Empowerment, vigilance, recognition, and celebration are the cultural cornerstones. The change has enabled the hospital to increase its market share, increase its satisfaction rankings, increase its employee retention, and enjoy consistent financial success.

By 2004, only organizations whose institutional operating strategies are built on a continual state of readiness and include performance improvement practices throughout the organization are going to successfully meet Joint Commission on Accreditation of Healthcare Organizations standards. As stewards of patient care, nurses maintain a unique role in identifying and guiding the intervention processes central to quality care, which prepares them to become key players/designers of a paradigm that demonstrates commitment to establishing and maintaining quality care. However, without recognition and support from organization leadership and physicians, the opportunity to effectively use the capabilities of nursing may be lost. The collaborative perspectives offered here attest to the fact that mutual belief and vision, coupled with creativity, strategic planning, and implementation, can effectively mobilize resources to establish priority measures and achieve quality patient/safety outcomes within the organization. Shifting the paradigm from just meeting the standards to continual readiness and performance improvement throughout the organization then becomes mission and mantra.
