The roots of Kaiser Permanente go back nearly 70 years to the building of the Los Angeles Aqueduct, Washington's Grand Coulee Dam, and the West Coast shipbuilding efforts of World War II. The key elements of managed care organizations today—prepayment, preventive care, and group practice—were part of the founding principles of Kaiser Permanente. This article provides a historical perspective of how Kaiser Permanente evolved from these roots to become the largest nonprofit health maintenance organization in the United States. It further describes Kaiser Permanente's fully integrated health care program and offers pragmatic examples of how integrated care is delivered.

The journey to a productivity management system entails the implementation of a core of four systems. Each system is independent yet interdependent on the others. Patient classification is the first step on that journey. Terms must be defined, policies and procedures established, forms developed, and consistent education applied. This is a complex project. Creating a network of different facilities all using the same system exponentially increases that complexity. That complexity is worth the effort because the outcome gives a capability to benchmark with other facilities, make decisions concerning professional resources based on data and choice instead of chance, and support nursing practice.

Maximizing the effectiveness of an information systems purchase is critical for every health care organization. This article addresses the overall process for evaluation, selection, and purchasing information systems. While not directed toward the selection of a specific software application, the paper focuses on the bedrock principles and processes for successful system selection results, including system requirements definition, the request for information and request for proposal process, product demonstrations, reference site visits, and contract negotiations. Specific attention is given to the importance of defining software application requirements around the business and operational needs being automated. Specific tips and procedures are offered, along with essential information about the current vendor industry.

The frequent and widespread dissatisfaction with a patient classification system (PCS) often can be attributed to its expected, singular application: satisfaction with staffing. Utilizing systems theory as the framework to analyze the historic dissatisfaction with PCS results in the development of comprehensive and reliable PCS and staffing systems. Recognition of the independence of each of these systems, as well as each system's design components of structure and process, results in many significant outcome (patient data and staff data) applications for each of these systems. Hospital-specific successes are identified, which illustrate the value for the nurse executive when assuring that the PCS is not viewed as the “proxy” for staff “busyness.”

An information system selection process offers a unique opportunity to foster improved communication and understanding among hospital departments, especially as they relate to information needs, language differences, and work processes. Because nurses are integral to the functions of nearly all departments, their presence on system selection teams should be aggressively pursued. The consequences of not including nursing on selection teams are potentially dire. Participation in the computer system development also helps each department and the information systems department work together to gain buy-in and enthusiasm for the services of the system. Nurses themselves should want to participate in the process because in the long run, a well-informed, culture-aware, decision-making process results in a better work environment and improved processes.

When patient satisfaction dropped to the 40th percentile group at the East Jefferson General Hospital in Metairie, Louisiana, staff, physicians and administrators believed adding staff would restore the hospital's award-winning status. How many employees should be added to recover the hospital's historical high standard of service?In partnership with a consulting firm, a system was implemented that measures and correlates patient satisfaction, quality, and cost in real time. The findings revealed that more staff does not necessarily equal higher patient satisfaction scores. Award-winning hospitals have achieved and sustained patient satisfaction scores above 90% without adding staff by keeping score with real-time data.
