摘要:
Stressors intrinsically associated with nursing provide a desirable moderate level of stress leading to high job performance. More (or less) stress would likely lead to a drop-off in performance. A large, urban hospital's organization has been designed to minimize further stress arising from its organizational culture. Its dominant values are responsibility, high quality nursing, collaboration, supportiveness, caring, autonomy, clinical development, and professionalism. Expressed in Jungian terms, the organization embodies Great Mother, Anima, Wise Old Man, and Anima archetypes. Nurse executives and administrators who understand the effects of organizational culture on stress may help their institutions operate in stress's healthy midrange.

The rural acute care hospital used to be a simple organization before regionalization and integration. Creating a shared vision including interactive planning and strong leadership provides an effective framework for dealing with change and turbulence. Central to dealing with stress and turbulence is the development of strong, trusting relationships with all care providers and disciplines. With strong community relationships within a community health care system, health status can be positively impacted in rural areas.

The nursing profession must continue to cement recognition as a powerful and innovative force in health care by first “caring for themselves” at the bedside for the goal of patient continuity of care. The negative effects of stress, both internal and external, major, minor, and adaptive, are rooted in the nurses occupational stress level, and how that level influences the continuity of care. Stress assessments and unit specific stress management sessions need to become ingrained within the health care environment by instituting “comprehensive” stress management programs. Communicative “healing” needs to flow from the bedside to the CEO and back down again.

Stress among chief nurse executives (CNEs) can result from the desire to meet work-related responsibilities and maintain a satisfying personal life. The purpose of this research was to determine if the stress level that results from balancing work-related and personal-life time pressures differs among CNEs by gender. CNEs experienced moderate levels of strain; gender differences were apparent. The research findings demonstrate that the influence of gender within a female-dominated profession is consistent not with the gender norms of females, but with the gender norms seen within the larger society.

Very little research exists regarding qualities that characterize an excellent head nurse (HN). Sixteen nurses were asked to tell the researcher about an excellent HN that they had worked with at some point in their career. The excellent HNs could be characterized as having a humanistic management philosophy and being skilled at negotiating and conflict resolution. These leaders have excellent interpersonal skills and are powerful advocates for nurses and patients. They are viewed as highly credible, innovative and even tempered. Generally, these leaders were able to meet the needs of the nurses—be they clinical, professional, or personal.

The Covey Time Management Matrix provides a framework for organizing daily tasks according to urgency and importance. Important activities contribute to one's goals. Managers must spend some time on the urgent, important aspects of their job. However, those who spend time in the nonurgent activities important to their goals are less stressed.

As nurses increasingly turn to teamwork as a viable option for accomplishing a myriad of duties and responsibilities, they are discovering that teamwork can also be a source of conflict and stress. This article suggests that conflict is both inevitable and necessary within a team. A model of conflict is presented that aims not at reducing conflict, but at making it less distressing and thus more effective. The authors trace the influence of pre-conflict conditions and perspective-taking on conflict and ultimately team effectiveness. Finally, to apply the conflict model to practice, two examples are presented demonstrating both an ineffective and an effective use of conflict.

A formal computerized way for nurses to practice intraprofessional consultation was developed by envisioning the ideal and using the research process. Results of the evaluation were: (1) informal consultation per phone or face-to-face continued as before; (2) formal nurse-to-nurse consultation through the computer was rarely being used; and (3) documentation of nurse-to-nurse consultation continued to be rare.

Stress has become a significant part of the daily lives of health care administrators, yet little attention has been paid to their experience. To survive in today's health care environment, administrators must address the effects of stress and develop healthy stress management responses. While many traditional stress management techniques may be effective, humor is a naturally occurring phenomenon as variable and individualized as stress. Humor helps us make sense of, understand, and cope with reality and serves as nature's biofeedback, stress-control system. Humor plays an important role in stress management and its use should be recognized and encouraged.
