It is possible to slow down noticeably the development of the disability in patients with myopathy of the DDB I Type by a physical therapy treatment regimen consisting of vasodilatation. So far our experience indicates that this slowing down can be as much as 4 to 5 years. The treatment should be undertaken as early as possible and should be done daily and continuously. One can also avoid the development of contractures by hydrotherapy treatment and passive stretching and by systematic application of night splints. Early start of the treatment markedly improves the results obtained. The very early diagnosis based on the determination of creatine kinase level in the serum done at birth or in the earliest months of life therefore becomes particularly important in the management of these patients. In themselves the results obtained in treatment are a new argument in support of the theory of microcirculatory origin of the symptoms of this disease. The. evaluation of our treatment results therefore supports the other physiological, histological and biochemical findings which have recently been presented by several authors in support of this hypothesis.

An accelerographic method using harmonic analysis to evaluate gait has been described. The results of the harmonic analysis were converted to a harmonic ratio which provided an index of smoothness for the acceleration curves. This method was capable of discrimination between the sub group classifications in eluded in this study. For vertical and fore all accelerations, the rank order of the harmonic ratios from highest lo lowest are: 1) 4.46 and 3.38 for normal adults walking at a moderate velocity: 2) 3.16. and 2.90 for normal boys; 3) 2.88 and 2.40 for normal adults walking at a slow velocity; 4) between l.34 and 1.91 for normal adults with induced defects and subject with a short leg cast; 5) between 0.86 and 1.89 for abnormals; and 6) between 0.85 and 1.47 for subjects with imposed crutch gaits. From the data obtained for this study, the following classification system for harmonic ratios might be used to indicate the smoothness of walking: normal equals greater than 2.00; fair equals 1.50) to 2.00; poor equals 1.00 to 1.49; and very poor is less than 1.00.

Four patients with seven post-operative Sharrard transfers received electromyographic study of their transfers. Four transfers had normal EMG activity; three had greatly reduced activity.
