One hundred patients with surgically proven ruptured lumbar disks were studied in terms of their diagnostic work-up. Four studies—electromyogram, CT scan, myelogram, and epidural venogram—were evaluated independently and in combination. No one study was consistently abnormal; however, epidural venography was diagnostic in 74% of cases. CT scanning and electromyographic examination, when used in combination, were found to be reliable in confirming the diagnosis of lumbar disk rupture in 84% of cases. In no instance were all preoperative diagnostic studies normal when a diagnosis of herniated nucleus pulposus was proven surgically. It seems, therefore, that a detailed history and physical examination in combination with a positive result in at least one of the diagnostic studies appears sufficient to establish the diagnosis of lumbar disk rupture.

We evaluated 101 children for hypertrophic pyloric stenosis (HPS) at Arkansas Children's Hospital. Diagnosis was based upon a palpable epigastric “olive” characteristic of HPS, or sonographic measurements of the pylorus. In 28 children, the diagnosis was confirmed by physical examination alone; the remaining 73 had ultrasound examination. Pyloric measurements included muscle thickness, transverse diameter, and total length. Pyloric function was also studied during examination. Using a thickness of less than 4 mm, a diameter of less than 13 mm, and a length of less than 17 mm as normal criteria, 31 cases of HPS were correctly diagnosed by ultrasonography. In the remaining 42, no pyloric abnormality could be demonstrated on further evaluation.

The diagnosis of systemic mastocytosis without urticaria pigmentosa has been made with increasing frequency since modern methods of histamine assay have been used clinically. We examined the incidence of urticaria-angioedema and mastocytosis over a recent 12-month, period. Of 490 new patients we saw, 52 had urticaria-angioedema, and ten had evidence of excess histamine ± PGD2, with at least ten mast cells per high-power field on skin biopsy. The average age was approximately 35 years; the male:female ratio was 1:4 for urticaria-angioedema arid 1:2 for mastocytosis. Symptoms of mastocytosis included flushing, abdominal cramping/diarrhea, syncope, urticaria-angioedema, pruritus, and headache. Symptoms have typically been prevented by a combination of H1and H2antagonists, with addition of a cyclo-oxygenase inhibitor in syncopal cases. Acute hypotension has responded to epinephrine.

Of 105 admissions to a medical service, 64 involved anemia (defined as a hematocrit value of less than 38%) at some time during the admission. Chart review revealed that the disease concept was not being used, as most of the anemias were unnoticed or undiagnosed, and most of the diagnoses that were made were incorrect.

The binge-purge syndrome (bulimarexia) is one of the four major eating disorders. It is a very important entity with which all clinicians should have some familiarity. This is true not only because of its frequency and the potential severity of several of its numerous complications, but because it has become one of the common masqueraders of modern medicine. Indeed, nephrologists may become interested in and involved with patients having this disorder because of their frequent fluid-electrolyte and acid-base disturbances, particularly hypokalemia and metabolic alkalosis, which initially often cause considerable diagnostic confusion. This paper presents an overall view of the binge-purge syndrome, with focus on several of its nephrologic aspects.

This review of the causes of erectile dysfunction and the salient points involving the history and physical examination outlines the studies necessary to distinguish between organic and psychologic causes and to further delineate the specific organic causes. Various types of treatment for organic dysfunction, such as oral, intramuscular, or intracorporeal pharmacologic manipulation and artificial external penile devices, provide alternatives to internal prostheses. The types of prostheses available and the complications associated with each are thoroughly reviewed, followed by a brief outline of the future for the treatment of erectile dysfunction.

This conference concerns economic, psychosocial, preventive, and medical aspects of the care of an indigent, unemployed, 22-year-old mother of three who died of an invasive, large cell, nonkeratinizing cervical cancer 35 months after her last Pap smear, 19 months after the onset of vaginal discharge, 12 months after consulting a physician, 10 months after an exploratory laparotomy, nine months after initiation of radiation therapy, five months after performance of a colostomy, four months after initiation of chemotherapy, and three months after treatment of small bowel obstruction with hyperalimentation and resection. We discuss the cost effectiveness of preventive programs.
