Diagnostic compound B-mode ultrasound scans done on gynecologic patients during the past three years were reviewed and compared with surgical findings or clinical follow-up. The 190 scans were analyzed and categorized as diagnostic, compatible (or confirmatory), equivocal, misleading, or negative. Although only 6.4% were diagnostic, the majority (66.3%) were compatible with the clinical and pathologic findings. Criteria for interpreting B-mode scans are discussed.

Five patients who presented at the Veterans Administration Mental Hygiene Clinic, Brentwood Hospital, Los Angeles, California, are described. All claimed to be transsexuals and requested estrogens and sex reassignment surgery on admission. They created great difficulty at the time of their admission and follow-up because of the lack of precedents for such eventualities in government medicine. These veterans, displaying a mosaic of psychologic problems, were followed up for a period of three years. The federal social medicine system, particularly the VA, is not equipped psychologically or physically to deal with unusual requests.

Symptoms of disorders in taste and smell range from loss of the sensations to an increased sensitivity. Causes include infections, metabolic disorders, and central nervous system disturbances. In many instances, the diagnosis for these disorders is obvious, such as an upper respiratory tract infection. In some cases, the diagnosis eludes the clinician and no pathologic condition is found. For the idiopathic taste disorders, zinc therapy is available; however, no therapy exists for idiopathic smell disorders.

Detailed immunologic studies were done on 29 patients with colorectal cancer. The plasma level of circulating carcinoembryonic antigen, the in vitro reactivity of the peripheral blood leukocytes (PBL) to colorectal-tumor-associated antigens (CTAA), and the competence of the T cell population were determined. The in vitro reactivity of the PBL to CTAA was determined by a lymphoblastic response and leukocyte migration inhibition. The competent T cell population was determined by enumerating the T and B cells, the rosette-inhibiting titer of antithymocyte globulin, and the reactivity to skin test antigens. An arbitrary score ranging from 0 (low immunocompetence) to 100 (high) was assigned to the results of each test. The mean score or the immunocompetence quotient (ICQ) which ranged from 22 to 100 was judged to reflect the immunocompetence. The sequential ICQ of individual patients strongly suggested that this information reflected the immunocompetence of patients with cancer of the colon.

This paper reviews the otolaryngologic manifestations of acute leukemia and lymphoma. Our experience includes 50 patients with acute leukemia and 61 patients with lymphoma of the head and neck. Acute leukemia was manifested by infection, bleeding, tissue infiltration, and complications of chemotherapy. Lymphoma manifestations were usually head and neck masses. The otolaryngologist must be familiar with the manifestations of these disease entities.

A technic incorporating the simultaneous elevation of multiple adjacent thoracoabdominal pedicle flaps, based in opposite directions, has been successfully used on four patients with extensive degloving injuries of the hand. In all cases, the hand was later divided into individual digits. A five-year follow-up of three patients is presented, illustrating functional results good enough to allow the patients to return to work. The special problems of a circumferential wound and digital shortening appear to be better controlled by this technic than by a single long “wrap-around” flap. Various technics of skin coverage are reviewed.

To define the incidence and possible significance of new arterial hypertension during myocardial infarction, the records of 87 consecutive infarct patients were examined. Forty-two patients had a history of previous sustained hypertension and were deleted from the study. Of the remaining 45 patients, one was omitted from consideration because of vasopressor therapy for severe hypotension on admission. The study group thus consisted of 44 patients without a history or physical findings of previous hypertension. Nineteen (43%) of these 44 previously normotensive patients demonstrated acute arterial hypertension (blood pressure greater than 140/90 mm Hg) averaging 165/106 mm Hg and persisting for 9 ± 12 hours. The remaining 25 patients had blood pressures averaging 122/76 mm Hg. Infarct location in the 19 acutely hypertensive patients was judged electrocardiographically to be anterior in nine and posterior in ten. Eleven of these 19 patients subsequently had coronary arteriograms. Ten of these 11 demonstrated severe proximal stenoses in the left anterior descending or left circumflex coronary arteries, regardless of location of the infarct. On comparison to our entire series and to the experience of others, we found that new arterial hypertension is more frequent (nearly half our cases) than previously recognized, that it lasts only some hours, and that it does not seem to affect prognosis. The occurrence of such hypertension does not correlate with electrocardiographically defined location of the infarct, but does have a high degree of correlation with severe disease in the proximal left coronary circulation. The possibility that the acute arterial hypertension was mediated by a chemoreceptor near the main left coronary artery is discussed.
