The atypical mycobacteria are identified readily when appropriate attention is paid to growth rates, pigment formation, and temperature requirements. They are widely distributed in nature with approximately 40 million of our population having been exposed to them as indicated by skin testing surveys. Various disease patterns are produced—pulmonary infection, lymphadenitis, cutaneous involvement, and less commonly, others. Organisms from all four of Runyon's groups may produce cutaneous infection, but the majority are caused byMycobacterium marinum, which usually produces a granulomatous verrucose lesion, but at times may simulate sporotrichosis. Group 2 and group 3 organisms rarely affect the skin. Group 4 organisms typically produce subcutaneous abscesses following injections or trauma. Most atypical organisms show some resistance to the major antituberculous drugs, and sensitivity testing is necessary to determine chemotherapeutic agents of choice.

Purulent pericarditis is a secondary disease process which can be, and usually is, masked by the clinical findings of the primary infectious disease. Prompt diagnosis, therefore, depends primarily upon the clinician's high index of suspicion of the possibility of coexistent purulent pericarditis in a patient being treated for another infectious disease whose condition suddenly begins to deteriorate. Once suspected, the diagnosis can be confirmed quite readily by physical examination, chest roentgenography, and pericardiocenresis. The treatment should begin with appropriate antibiotic therapy and be followed as soon as possible with left anterolateral thoracotomy, partial pericardiectomy, and drainage of the remaining pericardial space via the left pleural cavity.

Many workers have noted stability of the cardiovascular system in patients receiving droperidol-fentanyl. We investigated the circulatory and respiratory influence of the narcotic analgesic fentanyl and the neuroleptic droperidol in 15 young, healthy volunteers. The variations of arterial pH, Pco2and Po2were in the direction of respiratory acidosis and hypoxemia when fentanyl alone or droperidol-fentanyl was used; however, the values obtained were still within accepted normal limits. The effects on the total peripheral resistance were manifested as a significant decrease when droperidol alone or droperidol-fentanyl was employed. No depressant action on the myocardium was observed.

Effectiveness of 8–0 chromic collagen suture in ophthalmic microsurgery was evaluated experimentally and clinically. In vitro and in vivo measurements of the physical properties of the material were made using rabbit corneas, and the suture was used in closing corneoscleral sections in 101 routine human cataract operations. Suture diameter ranged from 0.055 to 0.065 mm; average rupture strength was four times that of 10–0 Ethilon and almost twice that of 8–0 silk. Sutured rabbit corneal wounds tolerated intraocular pressures of over 250 mm Hg before rupturing. Over 80% of all sutures used in rabbits, and 95% used in humans, remained in place after two weeks. In the human cataract operation, no operative complications directly attributable to the material, and only six postoperative complications possibly related to the suture or wound approximation, were observed. It was concluded that 8–0 chromic collagen is an acceptable and welcomed addition to the armamentarium of ophthalmic microsurgeons.

Congenital hernias of the diaphragm present in two distinct forms: acute pulmonary insufficiency in the newborn, and chronic recurrent respiratory disease in older children. The high mortality rate in the neonate is due to hypoplasia of the lungs, resulting in respiratory acidosis and hypoxia. In older children herniation of viscera probably occurs after complete expansion of the lungs. Preoperative management includes correction of metabolic acidosis, maintaining normal body temperature, and, if necessary, assisting ventilation via an endotracheal tube. A subcostal abdominal incision has been used for left diaphragmatic defects; only right diaphragmatic hernias are approached by thoracotomy. In the eight-year period, 1964 to 1972, nine infants and children have been treated with only one death,—an 89% survival rate.

A patient with staphylococcal infection of an aortic valve prosthesis was desensitized to penicillin after failure to respond to alternate antibiotic regimens. Positive skin tests to benzylpenicillin were predictive of systemic anaphylaxis that occurred during desensitization. Prompt pharmacologic treatment of this reaction allowed penicillin therapy to be continued.

Primary resection is the treatment of choice for patients with melanoma involving the nose and paranasal sinuses. Multiple surgical approaches are available. The technic of extended lateral rhinotomy allows resection involving the nasal cavity, antrum, and frontoethmosphenoid complex. Three cases are discussed in which tumor was removed by this approach and melanomatous tissue involving the cribriform plate was treated by electrocoagulation. Morbidity was minimal, and the patients were able to continue a productive existence, free of pain, after operation.

SGPT, SGOT, and SOCT analyses were made before and after operations. Increasing enzyme levels indicated changes in hepatic function. Enzyme increases which occur postoperatively on the day of operation are associated with upper abdominal operations. Increases seen on postoperative day seven are associated with all types of operations. There were no differences observed in enzyme patterns between halothane, methoxyflurane, or spinal anesthesia. There is evidence that enzyme increases are of greater magnitude after the halogenated anesthetics than after spinal anesthetics. The results were quite different with cyclopropane as there were no abnormal levels after this agent nor were there rising trends on postoperative day seven as with the other anesthetics.

The high morbidity and mortality associated with orocutaneous fistula following laryngectomy have prompted a thorough reevaluation of technics designed to reduce the incidence. All of the previous technics are reviewed and a suggestion of a new technic to reinforce the pharyngeal suture line is described. This technic has been quite successful in a small group of patients.
