Pulmonary diffusing capacity (DLCO) and pulmonary capillary blood volume (Vc) was measured in 11 male volunteers at rest and during exercise at 22 C (71.6 F) and after two hours at 2 C (35.6 F). Resting DLCOrose from 33.8 to 37.7 ml/min per mm Hg (p <0.001) after exposure to cold. This increase was mediated through an increase in Vcfrom 88 to 104 ml (p <0.001). Exercise DLCOand Vcwere not significantly different from control. It is suggested that at rest in the cold, blood is shifted from the cutaneous circulation into the pulmonary circulation, while during exercise in the cold, cutaneous flow is reestablished.

Diagnostic cervical cytologic and histopathologic study of 81 patients was done by the Papanicolaou smear, punch biopsy, and mini-cone biopsy methods. After initial experience with the mini-cone instrument, sufficient circumferentially coned tissue for histopathologic study was regularly obtained. The mini-cone procedure using the disposable plastic device provided information equal to that of the punch biopsy in 76 of the 81 cases, which were approximately equally divided into class 1–2 smears (negative) and class 3–4 smears (dysplasia). Only the mini-cone biopsy revealed unexpectedManilia(one case) and “contraceptive” hyperplasia (three cases). No bleeding problem was encountered. The mini-cone information obtained was at least equal to that obtained by punch biopsy, a procedure often used to supplement Pap smear findings. If these preliminary results are supported in a carefully structured protocol led study, consideration could be given to using the mini-cone biopsy method when the Pap smear needs to be repeated.

Thrombotic thrombocytopenic purpura (TTP) is a syndrome in which thrombocytopenia, red cell fragmentation, and variable neurologic signs and symptoms are the defining features. Variations in the presentation and subsequent course of the syndrome are well known; two case histories illustrate possible variations. The role of fibrin formation and the relationship of TTP to other forms of vasculitis and disseminated intravascular coagulation are discussed, with emphasis upon appropriate selection of treatment.

An incontrovertible full-term ovarian pregnancy and nine other ovarian pregnancies are described. Pathologic aspects of such cases are stressed, with emphasis on the difficulties in gross and histologic documentation of those rare cases which proceed to term. Conservation of normal fallopian tubes and preservation of the afflicted ovary by resection of the pregnancy, if feasible, are recommended.

Pulmonary function, oxygen uptake, and oxygen debt were measured in female smokers and nonsmokers 19 to 39 years of age before and after a month of repetitive treadmill exercise. A significant decrease in vital capacity, airway resistance, and pulmonary diffusing capacity was found in the smokers compared with the nonsmokers. Oxygen debt after five minutes of exercise at 3 mph on an 8% grade was significantly less in nonsmokers than in smokers before training. Both oxygen debt and the ratio of oxygen debt to total increased oxygen uptake improved with training in smokers and nonsmokers, suggesting an increased efficiency of oxygen delivery to the tissues.

The healing reaction of a child after a severe traumatic ocular injury differs from an adult's reaction to a similar injury. Because the child reacts differently, an altered surgical approach is required. Surgical technic and postoperative management must take into consideration the excessive fibrosis which can become, and often is, the one factor that prevents good visual results in children. Correct surgical management of the lens, the vitreous, and the iris are important in preventing fibrotic overgrowth. Other problems requiring particular attention in children with traumatic ocular injuries are tight wound closure, retinal detachment, and postoperative management.
