摘要:
A case study, resulting from a naturalistic inquiry, describes the inpatient occupational therapy experience of an adult woman with an eating disorder. The details that emerged as the study unfolded through observation and interview allowed the inquirer, as well as the respondent, to make sense of the respondent's illness and the benefits of occupational therapy to her health while a patient, and following her discharge. The patient elaborated about the importance of her relationship with her occupational therapist, and provided details about task and process groups that demonsirate the efficacy of these groups to the positive outcome of hcr trcatment.

摘要:
The significance of program site for inpatient therapeutic activities offered to chronic patients is rarely discussed. This paper reviews experiences with the development of a facility-wide off-unit activities program in a state psychiatric center. The organizational, clinical, and administrative implications of off-unit proramming are examined in relation to on-unit treatment ractices. iecommendations are made to assist the successful introauction of off-unit programming and the identification of the most effective location for specific activities.

Deinstitutionalization as a social change phenomenon has had a profound affect on both hospital andcommunity psychia- try and on the almost 2 million chronically ill, their families, and the communities in which they now live. Two individuals, Gail and James, were subjects for a study which asked three research ques- tions. First, how have these individuals with a chronic mental illness functioned as deinstitutionalized persons; second, did participation in a fifteen-week occupational therapy program affect performance of skills in work, play, and self care; and, finally, what is a model for the occupational therapis1 as case manager with the chronically mentally ill? Descriptive case study methodology revealed previous rehabilitation efforts over ten years were fragmented and time lim- ited and demonstrated that the subjects made periodic gains in self- care skills; but that these were not maintained in the absence of treatment. Pre- and post-testing of the subjects during the fifteen week occupational therapy intervention showed some specific skills gained and improved perfonance in the subjects' social skills in the home environment. The model for the occupational therapist as case manager describes the comprehensive nature of services required by this population which are particularly suited to the philosophy and practice of the profession of occupational therapy.
