We review the most recent findings regarding the drug treatment of mania, including issues about the role of lithium in acute and maintenance therapy, other adjunctive approaches, such as neuroleptics, benzodiazepines, and thyroid supplements and, finally, alternative agents such as carbamazepine, valproate, and calcium channel blockers.

Recent research relevant to the treatment of schizophrenia is reviewed. Of particular interest are reports of novel dopamine receptors which appear to be related to the D2receptor. The potential implications of these are discussed. Also reviewed are trials of medication regimens, studies of the atypical antipsychotic clozapine and various treatment trials.

Family, adoption and twin studies point to important genetic contributions in alcoholism, panic disorder and Tourette syndrome in addition to many normal personality traits. Carefully designed linkage studies should be powerful enough to detect the location of some of the genes implicated in these conditions. The ethical issues raised in clinics offering predictive testing to families with Huntington's disease will probably become increasingly relevant to psychiatrists.

Several neurotransmitter systems are implicated in the processes of chemical intoxication and dependence. Separate genetic transmission of these may lead to different alcohol-related behaviour. Co-operation between various branches of scinecen will be necessary to enable integrated research into these processes.

Despite decades of anatomic, kinetic, and pharmacologic analyses of neurotransmitter transport across neuronal and glial plasma membranes, the identification of specific gene products responsiible for neurotransmitter uptake has lagged noticeably behind the elucidation of receptor and ion channel molecules. Within the past year, however, this situation has changed dramatically with the cloning of complementary DNAs encoding amino acid (GABA) and monoarmine (norepinephrine, dopamine, serotonin) transportes. In particular, the cloning of highly related cocaine and antidepressant-sensitive norepinephrine, dopamine, and serotonin transportes provides clues to structural diversification likely to underline differential drug sensitivity and provides new tools for the study of the bilological basis for drug addiction and neuropsychiatric disorders.

The family of G proteins play a key role in transmembrane signal transduction. A review of the literature suggests that there is sufficient data to consider abnormalities in G-protein coupling or content in many neuropsychiatric disorders. Whether such changes are primary or secondary remains to be elucidated.

Recent research relating to post-mortem findings in depression, and metabolic changes and alterations in receptor function are discussed, as well as studies examining the problems of prophylaxis. This review highlights important theoretical papers providing new perspectives on the experiental phenomena of temporal lobe epilepsy and the neuropthology of the schizophrenia-like psychoses, reports the changing views about the prognoses of both the first fit and the recurring seizure disorder, outlines novel therapeutic developments and draws attention to recent papers on limbic epilepsy and its neuropsychology, epileptic psychoses, the behavioural aspects of forced electroenchephalogram normalization, kindling pseudoseizures, epilepsy in children and old people and the effects of unemployment.
