Much has been published about the treatment of panic disorder since it was first defined in 1987.Developments in both pharmacological and psychological treatments are reviewed and the advantages and disadvantages of combining the two approaches discussed. Problems encountered in this type of research are also highlighted.

Pain research has begun to focus on acute pain in clinical settings.Nurse researchers have investigated factors influencing their responses to complaints of pain in adults and children. Medico-legal issues have begun to loom large, especially when court reports are required. Concerns are being expressed about the management of pain for patients with an opiate addiction as well as the use of opiates in benign chronic pain.

It is still unclear whether some abnormalities of sexual function are true disorders or reflections of societal views.Paraphilias, particularly those involving harm to others (e.g. paedophilia), are best treated by seeing how the sexual preference reflects more pervasive personality disturbance. The main advances have been in treatments for male erectile dysfunction and the integration of treatment methods.

Given that efficacy and efficiency represent the 'gold standard' now being applied to psychotherapy, it is important to focus on factors that impede progress.These may be summarized under the heading of resistance. A search of the literature yielded relatively little on this topic. The most promising methods and findings were in microanalytic studies of therapy change. The candidates for high yield in future research will be studies that link attachment and loyalty to early 'bad' objects with resistance to interventions in therapy that would otherwise lead to therapeutic change.

Multiple personality disorder remains a controversial diagnosis.Little progress has been made towards establishing a consensus for the use of the construct of multiple personality disorder. The intensity of the controversy did not decrease when the name was changed to dissociative identity disorder. The controversy centers on iatrogenicity, the delayed effects of childhood trauma, and the variable constructions of the repression and dissociation metaphors.

Three areas of eating disorders research were reviewed.First, advances in the conceptualization of body image and its measurement were considered. Second, studies examining the relationship of psychiatric comorbidity and the eating disorders were reviewed. Third, psychobiological processes that may increase risk of developing or maintaining an eating disorder or represent medical complications of an eating disorder continue to be an important research topic.

In several countries, it seems to be policy to strive for a mental health system dominated by variants of community care replacing hospital care and culminating in closure of hospitals.Scientific evidence shows that no form of community care for the seriously mentally ill can do without the hospital, but also indicates compellingly that hospital care can be replaced or avoided by community alternatives, although only partially so. The randomized controlled trials that have produced such evidence are reviewed. Emphasis is placed on the convergence of findings across trials, their generalisability and the degree to which community alternatives succeeded in avoiding or replacing inpatient treatment. The results of the review are confronted with the justified demand for an evidence-based mental health system.
