There has been increasing emphasis on developing educational and treatment strategies for common mental disorders like depression, anxiety and somatic disorders. The exciting challenge for epidemiology and health-service research is to link studies of classification, help-seeking behaviour, detection and outcome with strategies for education and intervention. It is unhelpful to outline the extent of the problem without developing and crucially, evaluating tools to deal with it.

Recent psychopharmacological and psychotherapeutic approaches to depression are reviewed. Individually tailored treatment is recommended in view of the biochemical heterogeneity of the illness and manifold psychosocial dimensions of the depressed person. Future research needs to further define significant variables and to fine-tune specific individual and combined psychotherapy/drug interventions.

Two research-based models of anxiety and anxiety disorders have appeared in the past year. They both emphasize that failure to cope is an essential aspect of an anxiety disorder. Two reviews of the relatively new syndrome, generalized anxiety disorder (GAD), have been published, one of which includes a meta-analysis of treatment-outcome studies. Lastly, two prospective studies of panic attacks and some dismantling studies of treatment confirm the importance of the cognitive view of panic, which emphasizes panic as a patient's response to fear about their increasing level of somatic symptoms of anxiety. Although it is possible to reliably differentiate between panic disorder and CAD at the clinical level, the aetiological, natural history or treatment-outcome data, however, have not yet been able to reliably discriminate between them.

Eating disorders continue to intrigue the medical profession. The recent literature covers a vast range of perspectives, from the cross-cultural to the molecular biological. Among the major themes are proposed revisions to classification, insights to be gathered from epidemiological studies, the recognition that there is still much to be learnt clinically, and further refinements of an already impressive literature on the treatment of bulimia nervosa (BN).

This paper examines the developments in behavioural-cognitive treatment of obsessive-compulsive disorder (OCD). The majority of patients with obsessive-compulsive disorder who embark on behavioural treatments have shown improvement. Recent research, however, has concentrated on those patients who fail to respond to exposure treatment or who refuse treatment. Other important issues concern the most cost-effective delivery of behavioural-cognitive psychotherapy.

Patients with somatoform disorders are more likely to be encountered in non-psychiatric settings, where they are often a financial burden on health services. The importance of further collaborative research with physicians and surgeons is emphasized. Special emphasis should be given to treatment studies which demonstrate the benefits and potential cost savings of psychological interventions.

Research has provided clues regarding the most appropriate strategies for reducing suicide rates. Deliberate self-harm remains a major cause of morbidity. The role of biological and psychological factors in these behaviours is slowly being elucidated.

Considerable interest in the role played by gender in neurotic disorders is displayed by the wide range of research and theory published on this subject in the past year. As usual studies about differences between genders or studies which focus on women's vulnerability predominate. Few studies, however, look at between-gender similarities (although some report these as 'surprise' findings) or concentrate on the vulnerabilities of men.
