The impact of neurotic disorders on society and medical practice are not fully evident from the perspective of current research in epidemiological psychiatry. Health services research and epidemiological enquiries require harmonization. In primary care settings, clinicians and research workers are focusing on longitudinal studies relevant to causes, courses and outcomes of neurotic disorders.

This article reviews a number of epidemiological research studies about depression. Several questions are examined: for example, whether research findings support or cast doubt on our current classifications; what the outcome is for those with depression; what effects treatments have on patients; and whether there are useful risk factors for the condition.

Suicidal behavior is an important health issue that caregivers must deal with on a very personal level. Researchers are continuing their efforts to evaluate its risk factors, cultural patterns, genetic and neurochemical underpinnings. The prediction and prevention of suicide remains problematic, but work continues in these areas.

The status of chronic fatigue syndrome remains controversial. Recent phenomenological, virological, immunological and epidemiological studies are reviewed with this issue in mind. There is considerable overlap between chronic fatigue syndrome and other psychiatric and non-psychiatric syndromes. Research has not yet firmly established any unique characteristic of chronic fatigue syndrome. A pragmatic rehabilitative approach to patient management and caution in the use of novel treatment is suggested.

Papers on the diagnosis and management of psychiatric and psychological problems presenting physical complaints (somatization) are reviewed. These include reports on conversion disorders, hypochondriasis and factitious disorders (Munchhausen’s syndrome). The need for a more phenomenological approach to these conditions is discussed.
