Until recently, little attention had been paid to the study of child and adolescent mental health practice in primary care. In the past year, an extensive review was published that summarized knowledge and practice in the area. Further work has detailed the relevance of child and adolescent mental health problems for primary care across different countries and age groups. In addition, a number of treatment initiatives have been successfully piloted.

Community and epidemiological studies show that the lifetime prevalence of major depressive disorder increases from puberty to late adolescence. There has been a tendency to interpret this varying lifetime prevalence to mean that depression is rare in childhood and increases in frequency after puberty. A critical re-evaluation of existing studies, however, finds no evidence of age-related changes in the current prevalence of depression in boys and little evidence for puberty-related changes in girls. Thus, the incidence of depression appears constant for boys across the child and adolescent age span but increases for girls. This review will focus on age-specific prevalence estimates of depression from community and epidemiological samples.

The rates of fatal and nonfatal suicide attempts by young people are increasing and have focused attention on this group. High levels of psychiatric morbidity, particularly depression and behaviour problems, have been identified and highlight the need for careful monitoring of young suicidal individuals.

Advances in the understanding of risk factors and other influences on adolescent substance use, abuse, and related behaviors are reviewed. Among the areas for discussion are family and peer influences, psychopathology, temperament, suicidal behavior, drug use and trafficking in the context of inner-city, African-American youths, and recent prevention interventions.

Post-traumatic morbidity, including post-traumatic stress disorder, in children and adolescents attracts increasing attention in relation to diverse stressors. Continuing clinical description and research are needed to clarify the range and course of post-traumatic reactions, the developmental variation of symptoms and the factors influencing individual vulnerability. The development of clinically relevant screening methods, preventive strategies following traumatic events and evaluation of treatment responses is critical.

Developments in the understanding of the psychosocial and cognitive factors associated with childhood chronic fatigue syndrome are reviewed. Treatment strategies are explored. A need exists for more community-based research in this area.
