In the introduction, the importance of insight as a key factor in determining treatment adherence and thus outcome is emphasized.Next, we briefly review how the concept has been defined and the recently developed instruments to assess insight. Advances in the differentiation of insight deficits in schizophrenia from those in other disorders are described. We review the data which suggest that neurocognitive deficits may be associated with poor insight in schizophrenia. The relationship of insight to psychotic symptoms is briefly addressed, with a description of one of the first attempts to study the factors associated with acting on delusions. This work suggests that delusional action may be related to the patients' attempts to test reality. Recent findings of associations with gender and ethnicity variables are mentioned. Studies examining medication-refusal and factors associated with noncompliance are reviewed, and poor insight and noncompliance are linked to frequent rehospitalizations and worse outcome. Finally, new biobehavioural approaches are discussed along with the development of strategies to combat problems in treatment collaboration and to improve adaptation.

The specialism of forensic psychiatry, or more correctly forensic mental health, is in a process of self-definition both clinically and academically. The papers reviewed here not only contribute to our knowledge about the relationship between disordered behaviour and disordered states of mind, and the problems of attempting to manage that relationship, but also contribute to an understanding of the limits and the potential of this discipline.

This review focuses on a selection of papers on violent crimes and deaths, and on the association with ethnic, cultural and economic factors and the possession of firearms and drugs.The influence of mental pathology and personality deviation in aggressive behaviour is also discussed. Recent research on violent crimes has focused in a conventional manner on the racial and ethnic issues. More attention should be given to the role of economic and social destruction in areas with high rates of homicide and suicide. The importance of firearms in violent crimes and the effectiveness of strict gun control laws have been questioned. The relationship of mental illness to violence and crime has been addressed in the context of institutional and community settings. There is a need for improved care of imprisoned drug-users, and community programmes for developing independent living skills are also needed to avoid recidivism.

This review focuses primarily on several studies that have been conducted during the past year regarding conditions that may lead to disability and compensation.Differences are found between and within countries about how assessments are conducted and how disability is interpreted and compensated. Post-traumatic stress disorder continues to be a focus of attention for those claiming disability and compensation. Other diagnostic considerations are reviewed in addition to the concept of chronic pain.

The concepts, definitions and delineations of mental disorders have become much more standardized and uniform worldwide through the development of new classification systems:DSM-IV and ICD-10. The reliability of the diagnostic process has improved by the introduction of a number of well evaluated instruments. New data have been presented for some established scales designed to measure the severity of syndromes, and some new measures have also been developed for specific areas of psychopathology.
