Empirical studies of change mechanisms in psychotherapy are rapidly growing in sophistication.Worthwhile advances are being made in cognitive and behavioural approaches to anxiety disorders, eating and substance abuse disorders and depression. Models offering systematic integration of insights derived from a wide range of approaches and incorporating an explicit temporal dimension appear the most promising.

Studies published in the past 12 months have helped to establish the effectiveness of brief psychotherapies in the treatment of patients with somatisation disorders.In addition to initial treatment responses, some brief therapies have delayed or long-term therapeutic effects.

Researchers of short-term dynamic psychotherapy have been relatively active during the past year. This review provides an update on the efficacy literature for short-term dynamic psychotherapy as well as a brief review of recent findings concerning the process of change in it.

Brief therapy delivers gain.Gain needs to be measured symptomatically, in terms of quality of relationships and in cost, both direct and cost-offset. Few studies meet these criteria. Growth-curve analysis is being applied to follow-up, with implications for dose of therapy and maintenance of gains. More studies need to be done of planned brief therapies at the upper end of the 25 session mark and beyond.

The family therapy literature betrays an unhappy divorce.On the one hand, there is an accruing body of research showing the efficacy of family treatments for a range of psychiatric disorders, including the most severe. On the other, the 'orthodox' family therapy literature reveals a preoccupation with theory, currently 'postmodernist', which sits uncomfortably with the demands of health care systems for evidence-based, cost-effective therapies.

Recent advances in understanding and preventing boundary violations include; new ways of expressing prevalence; management of the offending therapist by supervision; treatment, rehabilitation and disciplinary procedures; delineation of risky situations for the therapist in the light of recent reviews of erotic and eroticized transference; the development of training to reduce the incidence of violations, particularly by increasing awareness of countertransference.

The role of biological factors in phenomena such as drug and alcohol dependence is beginning to be understood, as are the biological actions in the central nervous system of dependence-inducing drugs. Drugs considered in this review are alcohol, opiates, central stimulants and benzodiarepines. From the viewpoint of the role of hereditary factors in the aetiology of alcoholism, a type 1/type 2 classification has been proposed, one group questions the validity of the hypothesis.

This review examines the effects of alcohol on women's health.It discusses studies examining the association of lower mortality with moderate drinking, explores reports relating alcohol consumption to health problems among women, and alludes to the ancient Mediterranean moderate drinking norm in light of this current research.
