Advances continue to be made in a number of basic research and health issues relating to people with Down syndrome.Particular areas include communication skills in children, medical and psychiatric morbidity, ageing and adaptive behaviour and Alzheimer's disease. Recently published research is categorized into these areas and reviewed. Comments on future research issues are given.

Effectiveness is influenced by many factors that have been considered in recent studies: type, philosophy, and intensity of program; characteristics of the family and their level of involvement; and characteristics of the child.

Recent research on families underscores the multiple long-term challenges associated with adjusting to and caring for a son or daughter with a developmental disability. Both the informal and formal support systems have significant and pervasive effects on parental wellbeing. Similarly, culture and ethnicity exert influences on families through belief systems and culturally endorsed practices, which increasingly are being considered in the design, conduct, and analysis of family studies. The literature reflects an interesting divergence, with both more clinical case histories and more sophisticated multivariate and longitudinal analyses being reported.

The emergence of transition as an area of study in relation to secondary education and school improvement for students with mental retardation provides a fertile ground for future research and development.In fact, during the past year very little research was conducted. Research in the areas of student involvement in transition planning, parent involvement, collaboration, and educational curricula, and professional development is reviewed.

Excellent work in the detailed examination of hospital records from 19th-century asylums continues to provide data from which to draw conclusions about the demographics and actual practices within these institutions and to reassess the role and ethical stance of large asylums in society. The effects of deinstitutionalization and assignment of responsibility for the worsening neglect of the chronic mentally ill continue to be debated.

The problem of consent in psychiatry and related fields has been the subject of much recent debate.In most European countries, mental health legislation is presently under review. Issues surrounding capacity to consent, advance statements and euthanasia are the subjects of the present review. Although these issues are not wholly or even predominantly in the field of mental health, developments in these areas greatly inform the debate surrounding mental incapacity and the overriding of patient consent within mental health practice. Particular attention is drawn to the Law Commission's report and proposed new Bill on mental incapacity in the UK. An important issue highlighted within the debate surrounding capacity is assessment of competence. Assessment procedures need refining and updating and better quality control is needed on decisions of incompetency.
