Research into community care for people with intellectual disability has explored the determinants of effective service delivery in a wide range of settings. Diverse methodologies have been applied across differing levels of ecological analysis. The generalizability and usefulness of such studies remain critical issues in the development of this field.

Psychopharmacological treatment in mentally retarded individuals is severely hampered by the lack of appropriate diagnostic systems, the underestimation of atypical psychopathological manifestations and aetiologically specific psychiatric disorders and the virtual absence of methodologically reliable clinical studies. With respect to psychopharmacological strategies, some promising vistas emerge from treatment with opiate antagonists and serotonin-modulating compounds. Interpretation of the results, however, remains difficult because no real insight exists in the aetiopathogenesis of behavioural disturbances and because of the possible presence of comorbidity with psychiatric disorders or delirious states.

In the past year, research into psychiatric disorders among mentally retarded people have focused on the amelioration of psychiatric diagnostics, on developing new, as well as adapting existing, diagnostic instruments, on the behavioural phenotype specificities, on the emotional development of children with mental retardation, and on establishing mental-health-care services for the mentally retarded population. Different facets of these developments are discussed in more detail in this paper.

Significant advances relevant to the study of neurobiological mechanisms underlying mental retardation are summarized in this review. The advances include the identification of a mechanism linked to a proposed new form of X-linked mental retardation and to mechanisms underlying other known disorders, including phenylketonuria, Williams, Smith-Lemli-Opitz, Angelman, and fragile X syndromes. The implications of these studies are relevant to diagnosis, prognosis, and treatment of disorders. They are also relevant to making generalizations regarding neurobiologic explanations for behavior.

Recent advances in the area of maternal substance abuse have been made by an improved understanding of the mechanism of alcohol teratogenicity. Epidemiologic studies have provided some new insights, and growing experience with long-term outcome has added to the literature. There is a striking inadequacy of new studies on the prevention and treatment of maternal substance abuse.
