This paper focuses on the management of deliberate self-harm. Guidelines on the management of deliberate self-harm have been available for the past 6 years, but they are not widely followed, with less than half of patients having a psychosocial assessment after an episode of deliberate self-harm. The availability of paracetamol has been restricted (blister packs and limited quantities), resulting in a reduction in the dosage levels of paracetamol ingested, but not in the numbers of patients that need to be referred to tertiary services for treatment of liver damage. An important medicolegal issue has been raised concerning patients who refuse treatment after a potentially life-threatening overdose. Approaches to dealing with this situation have attracted considerable attention during the past year. Two new brief and focused treatments for the management of deliberate self-harm have been reported (Manual Assisted Cognitive Therapy and Cognitive Analytic Therapy), but further research is required before they may be used in routine practice.

Body dysmorphic disorder, a distressing and impairing preoccupation with an imagined or slight defect in appearance, is an intriguing and under-recognized disorder that has only recently been systematically studied. Recent research findings indicate that body dysmorphic disorder is relatively common, causes notable distress and impairment in functioning, and is associated with markedly poor quality of life. Available data on treatment response, while still limited, suggest that serotonin-reuptake inhibitors and cognitive-behavioral therapy are often effective for body dysmorphic disorder. Further research is needed on all aspects of this understudied and severe mental disorder.

Recent developments in the psychological and pharmacological management of post-traumatic stress disorder are reviewed. This review of controlled outcome studies indicates that: (i) cognitive behavior therapy is the psychological treatment of choice; (ii) different components of cognitive behavior therapy can be effective; (iii) eye movement desensitization and reprocessing is not as effective as cognitive behavior therapy; (iv) selective serotonin re-uptake inhibitors are the pharmacological treatment of choice; and (v) there is increasing support for nefazadone but not for cyproheptadine in reducing the symptoms of post-traumatic stress disorder. The need for increased treatment effectiveness and the integration of recent findings into clinical practice is discussed.

Empirical research to assist health professionals in the development of group therapy models, to guide treatment differentiation, and to enhance the understanding of group processes for patients with personality disorders is advancing slowly. Recent efforts to investigate the relationship between group process variables and outcomes are therefore valuable contributions. Important studies have been published on comprehensive treatment programmes, relying heavily on group therapies, for the more severely disturbed patients.

Numerous as they are in emergency psychiatry settings and hard to treat, suicidal patients with personality disorders present clinicians with extraordinarily difficult challenges. This review focuses on recent developments in our understanding of personality disorder in relation to suicidal behaviour, and on new methods and models for clinical management and treatment aimed at reducing suicidal behaviour in patients with personality disorders.

The co-morbidity of axis I disorders and axis II personality disorders is reviewed. As expected, co-morbid diagnoses in most studies yielded poorer outcomes. The early recognition of both axis I and axis II disorders, particularly in late childhood and adolescence, may prevent the later development of adult axis I and axis II psychopathology. The clinical relevance of the categorical versus dimensional classification of personality disorders is discussed.

The present review, which covers the literature on psychological consequences of road traffic accidents (RTAs) published since 1999, identifies three major areas of research in this field. The first area represents prospective research studies that longitudinally assess the progression of mental disorders, primarily post-traumatic stress disorder, after RTAs. The second examines the effects of RTAs on child and adolescent populations. Finally, recent developments in the psychological treatment of post-traumatic stress disorder among RTA survivors are discussed. Such research has identified promising as well as detrimental interventions that have been developed to treat this population.
