AbstractPhlebography of the foot was performed in 68 patients. In 20 of them there were suspicions of deep vein thrombosis of the lower limb. In six of these patients thrombosis of deep veins of the foot was revealed. One case of a superficial vein thrombosis was also found.

AbstractExperimental work using a wax phantom containing two calculi of differing size has shown that with tomographic sections 0·5 cm apart the smaller calculus was only visible on three or four cuts if a short (10–20 deg.) angle of swing was employed. This indicates that in clinical practice shortswing tomographic cuts about 1·5 cm apart should reveal small renal stones. Routine use of tomography on 97 cases of suspected calculi yielded five patients with more extensive calcification than was observed on the plain film and two in whom tomography revealed calculi where none had been seen on the plain film.

AbstractThe use of left ventricular cine-angiography in the left anterior oblique projection for the assessment of mitral valve stenosis is described. The closed valve in the milder cases appeared as a crescent-shaped radiolucent area. The more severe cases displayed distorted, narrowed openings and diminished leaflet motion.

The fact that this book has gone into five editions in 17 years attests, more convincingly than any review can do, that it is a valuable contribution to medical literature. Let it be said at once that it is excellent, and several sections are outstanding. For instance the chapter on the mucopolysaccharidoses makes a determined attempt to get some order into a somewhat chaotic subject, and is nicely up to date.

AbstractA new reference system for the densitometric measurement of bone mineralin vivois proposed. A solution of ZnCl2in water with the same effective atomic number as hydroxyapatite is described. The concentration is calculated from a modification of the equation of Spiers (1946) for determiningZeffof a chemical compound. The solution of ZnCl2has been tested experimentally and used in practice to measure the hydroxyapatite content of compact bone and spongiosa in the index metacarpal. A simple method of obtaining two radiographs in projections perpendicular to each other is utilized.

AbstractA single determination of the metacarpal cortical index can be considered accurate to within ±0·036, which is +10 per cent of the usual values of the cortical index in normal bone. Comparison of estimates of the over-all variation in the index (including observer variation) with estimates of the measurement variation indicates that much of over-all variability is due to the measuring process itself, and is therefore unavoidable. Improvement in reproducibility can be obtained by using the average of two independent determinations of the cortical index in place of a single assessment.

A case is described of bilateral ureteroceles lying within vesical diverticula. A review of the literature has revealed one previous case where a large simple ureterocele was located in a vesical diverticulum (Thornbury, 1963).M.H., aged 25, was admitted to hospital as an emergency with a one-week history of left loin pain and frequency of micturition. Two days prior to admission the pain became colicky in nature and radiated to the left lower abdomen. There was no pain on micturition. He had a past history of a leg infection, but no previous urinary symptoms.

The chemoreceptor system consists of groups of cells situated in close proximity to major blood vessels which act as the receptor elements involved in a reflex concerned with regulation of blood pressure and respiration. These cells are sensitive to changes in the hydrogen ion concentration, and the partial pressures of oxygen and carbon dioxide in the blood. They have a rich blood supply and sensory nerve connections. The major groups of these cells are found close to the carotid artery, the jugular vein and the aorta, but cells of this type are also found in relation to the vagus nerve and there are several other smaller glomera in the body.Tumours of these cells give rise to chemodectoma or non-chromaffin paraganglioma, which are comparatively rare. The most common site is in relation to the carotid body where about 600 cases have been reported.Mediastinal chemodectomas are much less common and up to 1969 only 28 cases had been described. The purpose of this paper is to describe one of these tumours arising in relation to the right side of the posterior mediastinum in a young pregnant woman. These tumours are generally asymptomatic and grow to large sizes before detection, but this case is unusual because it presented with neurological symptoms and was a dumb-bell type of tumour, partly intraspinal and part intrathoracic.

The clinical value of fistulography and sinography is well documented (Gage and Williams, 1943; Pendergrass and Ward, 1947; Dell, 1949; Goldfarb, Monafo and McAlister, 1964). It is common practice to introduce the contrast material under fluoroscopy. Where the fistulous openings are such that even the best instruments fail to prevent regurgitation of the contrast during injection, the “overcouch” method is a satisfactory alternative. Saxton and Strickland (1964) advocate its use particularly in the investigation of sinuses which are suspected to originate from bone infection, a situation in which films with sharp definition are required. An inherent drawback of this technique is the inability to make full use of fluoroscopic control during contrast injection. A rare case of primary splenic tuberculosis is presented here to record an unusual complication which might have been avoided if fluoroscopy had been used.S.Y.P., a male Chinese patient aged 63 years, presented in June, 1968 with a chronic discharging sinus in the left hypochondrium of ten months’ duration. He had been a known diabetic for the past ten years and between 1963 and 1965 was treated for pulmonary tuberculosis. Towards the middle of 1967 he developed a tender, hard, left hypochondrial swelling. This was incised and drained, but the wound did not heal. During the same period a similar swelling appeared in his left loin. It was drained and the wound healed spontaneously.
