129I is the longest-lived radioisotope of iodine (half-life 1.57×l07y). It occurs naturally, though in very limited quantities: the estimated world total from natural sources is ∼ 40 Ci. It is, however, produced in nuclear fission (in particular in power-reactors) and therefore likely to be released in significant quantities, mainly in gaseous form, during fuel reprocessing.

Prostatic carcinoma is the second most common cause of cancer death in the United States, although somewhat less frequent in this country. Its incidence would appear to be rising. A frequent incidental finding in the elderly, its prognosis in the younger age group can be poor.

Enthusiasm for treatment of localised tumours by intraarterial chemotherapy began in the late 1950's and gained impetus over the next ten years, but then largely declined, except for extracorporeal regional perfusion of the limbs for melanoma which was continued in a few centres. Recent years have seen a reawakening of interest which is reflected in the contents of this volume.

AbstractThe use of labelled bile salt retention is likely to be a major new technique in assessing the function of the terminal ileum. Papers to date have described two techniques to measure both the 100% and the seven-day retention values. These use whole body counters or uncollimated gamma cameras. We have compared the use of simple probe systems with an uncollimated gamma camera for the retention measurements. There was no significant difference in the results obtained with any of these systems. It is concluded that simple probe measurements can provide a precise and highly reproducible assessment of bile salt retention which greatly increases the availability of this test.

AbstractThe radiation safety practices in a group of 25 UK and USA hospitals have recently been assessed. This took the form of detailed inspections of some 62 medical radiation departments, including Diagnostic X-ray, Radiotherapy, Nuclear Medicine and Pathology/Research (Radionuclide) Departments. Empirical expressions called “Radiation Safety Indices” were devised to evaluate the incidence of personal doses and radiological incidents occurring from 1977–82 and to characterise the safety facilities, procedures, supervision and educational techniques in each department. An outline is given of national legislative material and voluntary codes of conduct, together with the results of the departmental inspections. The computed indices are presented graphically and an analysis given of apparent national trends.

AbstractConsideration of the clinical results reported, when a cyclotron produced neutron beam was used for treatments in the pelvis region, suggested that a constant RBE of 3 should not have been used for all neutron doses. Instead a variable RBE, which increased from approximately 3 to 8 (with decreasing dose), should have been used. Although some of these RBE values are much higher than 3, they have been observed in clinical practice. An “equivalent photon” isodose plan was produced by employing a variable RBE and, by taking a TDF limit of 86 for bowel, an isoeffect plan was produced. This shows that in the clinical situation under consideration much of the pelvis was overdosed. Doses to tumour cells and late effects are also briefly considered. It is suggested that, in neutron therapy, both an “equivalent photon” isodose plan and an isoeffect plan should be produced prior to treatment.
