In 1978 the DHSS funded the building of a self-contained two room diagnostic X-ray Unit above the Department of Radiology at King's College Hospital. The rooms were unusual as they were specially designed to allow X-ray sets to be installed and removed in a short space of time. The purpose of the Centre was to allow careful assessment of selected new radiological equipment, not only in respect of its physical performance, but also the operational features during a period of clinical use on patients sent to the hospital X-ray department for examination. The Unit, which is run jointly by the DHSS Scientific and Technical Branch and the King's Department of Radiology, became operational in November 1979. After some initial problems, it has got into its stride and it is now issuing a series of reports on equipment under evaluation. It is therefore timely to review the value of the information which is flowing from this activity.

AbstractFifty-three patients with proven tumours of the petromastoid have been investigated by high-resolution computerized tomography. They comprised benign and malignant epithelial tumours; primary and secondary cholesteatomata; neurogenic and glomus tumours; meningiomas and secondary neoplasms. High resolution CT has the advantage of showing both bone erosion and the soft tissue mass of the tumour. This important both for diagnosis and to show the extent of malignant disease in the petro-mastoid. The technique has become integral part of the X-ray examination of these patients and many respects has superseded other methods of investigation such as pluri-directional tomography and angiography.

The book, which consists of nearly 840 pages of text and figures covering the brain and spinal cord, is divided up into 16 chapters. Many of the chapters such as those on computed tomography, angiographic technique, the posterior fossa and the spine cover the subject in adequate detail, but others are thin on text and material and one or two such as the orbit are inadequate. There is a long chapter on meningiomas but nothing on facial trauma and little on hemisphere tumours.

AbstractRadiographs of the cervical spine in 11 patients with fibrodysplasia ossificans progressiva (FOP) have been examined, and abnormalities found in all. In 4, small cervical vertebral bodies with enlarged pedicles were seen in early childhood; later, variable fusion of the vertebrae was apparent in all patients. This fusion was first noted between adjacent neural arches in late childhood but also involved the vertebral bodies in some adult patients. It was only sometimes associated with ossification of the adjacent neck muscles, and appeared to be an important cause of neck stiffness in FOP. The radiological changes may be confused with those of Still's disease or of the Klippel-Feil syndrome.

AbstractEight divers who developed permanent scintigraphic bone changes following dives to various depths took part in a study to determine the nature of these lesions. Only one of these divers had radiologically evident bone changes in his scintigraphic lesion.Dynamic scintigrams of the bones containing the lesions were obtained from all eight divers and functional images generated of both amplitude (osteoblastic activity) and accretion (microvasculature) rate constants. All had increased amplitude in the area of the scintigraphic lesion indicating continuing osteoblastic activity. Three divers showed a decreased accretion rate in the lesions, indicating impairment or absence of local microcirculation; one of these was the diver who showed radiological changes prior to the study and the other two subsequently developed radiologically evident osteonecrosis.It is concluded that functional images, generated from dynamic bone scintigrams, can successfully predict dysbaric osteonecrosis.

AbstractThe possible mutagenic effect of microsecond pulses of ultrasound was investigated using 3 MHz ultrasound at diagnostic dosages, and also under conditions of increased pulse repetition frequency and acoustic power output. The first part of the study involved 5 strains ofSalmonella typhimuriumbacteria in which mutagenicity and viability were assessed using the Ames test, while functional competence was evaluated from microscopic observations of motility. No changes were observed in survival, in the incidence of mutants, or in microbial motility following irradiation with ultrasound intensities of 4.5 W/cm2at temperatures ranging from 37° to 43°C. In the second part of this study, the frequency of sister chromatid exchange (SCE) in Chinese hamster ovary (CHO) cells, treated with similar ultrasound dosages, remained unaltered when cells were irradiated at temperatures up to 41°C. At 43°C and above, cell division was arrested by hyperthermia, an effect unrelated to ultrasound.

I had been looking forward to its publication and, having read it, have not been disappointed.Paediatric clinicians are now using ultrasound as a screening investigation for many organ systems, as well as using it specifically to answer clinical questions. The experience of these eminent paediatric ultrasonographers, gathered together in a small book, is invaluable to any ultrasound department.

AbstractA fresh analysis of the data entered into the multicentre BIR fractionation trial of 3F/week versus 5F/week in radiotherapy of the laryngopharynx has been undertaken. Completed records of the 732 patients initially entered into the trial have now risen from 687 at the last report to 706. The data have been analysed in a manner similar to that adopted previously so as to measure the effects of the two regimes on both tumour and normal tissues, and some additional analyses have now also been made. There have been some modifications in the results in the various sub-groups which may be due to an inadequate number of patients having been followed up for long enough at the time of the previous analyses. More data for late radiation damage to normal tissues and new radiobiological findings have suggested possible explanations for the differences which have emerged between the two groups. The apparent differences between the sub-groups containing patients with highly localized tumours, which were reported in our prevous report, are now less marked and not statistically significant.

AbstractTwenty-seven early localized breast cancer cases in elderly patients, 21 stage I and six stage II, were treated with Tamoxifen only. In 15 cases there was complete radiological remission and in seven cases partial remission or growth arrest. Five cases did not respond to treatment. Tamoxifen seemed to be a useful alternative treatment for this group of patients. Possibly tumour mass and growth rate may affect the response to antioestrogen treatment.

This book could be useful to a houseman or registrar working in an oncology unit, but lacks the detail and balanced overall view of cancer therapy needed in a reference book. It gives a simple guide to cancer chemotherapy which could be disastrous if used as a DIY handbook by a “keen” general physician or surgeon. It is presented in four sections.The first deals with the strategy of cancer therapy, a multidisciplinary approach.
