摘要:
Last year the National Health Service spent approximately seven million pounds on the purchase of diagnostic X-ray equipment. This figure has increased by some 20 per cent per year for the past few years and shows every sign of continuing to increase.The choice of apparatus in any particular case is often arbitrary since little objective information is available upon which a rational choice can be made.

AbstractThree main anatomic variants of the left renal vein are present: pre-aortic, retro-aortic and circumaortic. Rarely, a supernumerary left renal vein may occur. The embryo-genesis of the left renal vein is reviewed. The significance of these anomalies, particularly the retro-aortic and circumaortic renal veins, in adrenal venography, renal vein renin sampling and shunting procedure for portal hypertension is discussed.

It is generally accepted that the anatomy of the adrenal veins is remarkably constant. In particular it is found that the left adrenal vein normally empties into the upper surface of the left renal vein just lateral to the aorta and vertebral column (Starer, 1965; Reuteret al., 1967; Kahn, 1971; McLachlan and Roberts, 1971). Furthermore, although there are countless variations in the inter-connections of the renal vein and the lumbar-azygos system (Fargasanu, 1938; Anson, Cauldwell and Pick, 1948), the left renal vein is usually a single pre-aortic trunk joining the inferior vena cava (IVC) at L1/2 level. This uniformity results from the comparatively unvarying pattern of embryological development of the IVC and its tributaries (McClure and Butler, 1925). However, in about 1·5 per cent of subjects (Davis and Lundberg, 1968), the left renal vein bifurcates to encircle the aorta on its passage to the inferior vena cava, while in 1–3 per cent of cases the left renal vein may be multiple (Kottra and Castellino, 1970). Chuang, Mena and Hoskins (1974a) discussing the embryology of the veins of the trunk have pointed out that the anomalies of venous anatomy are commoner than is usually thought. They have further shown (1974b) that such variations can result in difficulties in the catheterization of the renal or adrenal veins. We wish to report four further cases in which aberrations of the venous anatomy have complicated attempted left adrenal vein catheterization.

AbstractTwo cases of Waldenstrom's macroglobulinaemia are described. Lymphography was performed in both cases. The lymphographic appearances of Waldenstrom's macro-globulinaemia are indistinguishable from those seen in lymphomatous conditions.

During angiography of a mass in an ectopic kidney (crossed fused renal ectopia), it was noted that the inferior mesenteric artery arose entirely from the renal artery supplying the ectopic kidney. To our knowledge, the angiographic demonstration of this unusual arterial anomaly has not been previously reported.

Waldenstrom's macroglobulinaemia is a disease characterized by abnormal bone-marrow proliferation of the lymphocyte series, together with a raised level of the Immunoglobulin M fraction of plasma protein. The infiltration of a variety of organs, such as lymph-nodes, liver, and spleen, and occasionally other structures, by these neoplastic (monoclonal) cells, may give rise to manifestations which are appreciable radiologically. These have been enumerated by the present authors elsewhere in this journal (Bottomley, Whitehouse and Bradley, 1974).A form of amyloid disease may rarely be associated with Waldenstrom's macroglobulinaemia (Forget, Squires and Sheldon, 1966), as occasionally occurs with multiple myeloma (Bayrd and Bennett 1950). The present case is an example of histologically proven amyloidosis complicating Waldenstrom's macroglobulinaemia, and is the first recorded example of lymphography performed in this condition. In addition, there was subcutaneous calcification, which has been previously described in only one case with this combination of diseases (Case Reports of Massachusetts General Hospital, 1961).The patient, a 60-year-old male, presented in October 1968 with a three-year history of malaise and swellings in his neck and inguinal regions. On examination, the only abnormality was firm, discreet enlargement of the cervical, supraclavicular and inguinal lymph-nodes. Investigations at that time revealed: Haemoglobin 12·6 g per cent, massive rouleaux formation and ESR 130 mm/hour (Westergren); Serum protein 8·7 g per cent, albumin 2g per cent and globulin 6·7 g per cent with an increased level of IgM. Urine analysis showed massive proteinuria but no Bence-Jones protein.

The author is Professor of Surgery at Louisiana State University School of Medicine. In 121 pages he attempts to cover all aspects of the portal venous system. The ten chapters proceed logically, commencing with a lengthy account of the embryology of the system, and concluding with the medical and surgical treatment of portal hypertension. Surgical techniques are fully discussed and the book concludes with some experimental work.
