It has been said that history does not repeat itself, but that historical situations recur. Thus it may well be true that in years gone by there have been occasions in the world of medicine in which a wind of discontent has blown over the scene. But looking back, as I can do, over a span of 60 years, I do not remember a situation comparable to the one which confronts us in Great Britain today—a situation characterised by strong feelings of uncertainty and frustration—and in this year of crisis in general practice, the picture of a profession composed of angry men, young and old. The angry phase will inevitably pass. Similar manifestations of emotion have burnt themselves out before—as in 1911 when Mr. Lloyd George introduced the first national Insurance Act. We have to look therefore for the deeper causes of this new “winter of our discontent”, to discover, if we can, remedies which will preserve and enhance the historical traditions of medicine in both its public and its private image—medicine as a liberal, scientific, and learned profession. We have perforce to go outside the boundaries of our own little world—that microcosm which to us seems so large—and to take note of what is happening in the larger society of which we are a part.We are moving very fast indeed—and not only in space.

Theories so far advanced to explain primary protrusio are reviewed and an alternative hypothesis is proposed. An investigation to test the hypothesis is described in three sections.(1) The effects of stress on an artificial Y-cartilage in an adult pelvis were examined, and a distinctive alteration in pelvic shape recognised as resulting from this stress.(2) The pelvic shape in nine cases of primary protrusio and in eight cases of congenital dislocation of the hip was compared with the shape found in normal subjects. It was shown that the pattern of pelvic deformity detected in Part I is present in primary protrusio whereas the dislocated hips show a converse pelvic deformity.(3) The phenomenon of juvenile beaking of the Y-cartilage was examined. It was shown that the beaking was associated with a bulge of the acetabulum into the pelvis, involving sometimes the whole acetabulum, but more commonly its posterior half. The age incidence of this juvenile protrusion was determined radiologically and found to increase from birth to the age of eight and subsequently diminish.Cases are illustrated in which the juvenile bulge has failed to correct, leaving primary protrusio.It is suggested in the light of these findings that juvenile beaking is a true protrusion of the acetabulum, resulting from normal shear and rotation stresses without any associated bone disease; that this protrusion is normally reversible; and that failure of correction of it is the cause of primary protrusio acetabuli.

This ambitious work sets out to cover the entire field of all those practical procedures currently or recently in use in diagnostic radiology including neuroradiology, angiocardiography and gastroenterology. Although the major accent lies upon contrast studies in general, considerable space is devoted to special radiographic views, some of which may not be found in standard works dealing with radiographic technique.

The possibility of nephrocalcinosis developing in cretins, or following the treatment of cretins with thyroid hormone, is discussed, with a review of cases of cretinism with nephrocalcinosis which have been reported in the literature.A further case of cretinism with nephrocalcinosis is reported.

The epiglottis and the cartilages of Wrisberg are stated to be made of yellow elastic fibro cartilage and not to calcify as the majority of other laryngeal cartilages do. Negus (1949) states that the epiglottis in man does not calcify. He is quoted by Pressman and Kelleman (1955) who state in their physiological review that the epiglottis has never been known to calcify. Lederman (1964) states that he has seen a patient who showed plaques of calcification in the epiglottis following radiation treatment of the larynx: Wilson (1964) states that he has also seen flecks of calcification in the epiglottis occasionally. Baclesse (1938) when discussing, in considerable detail, calcification in the laryngeal cartilages states “Quant à l' épiglotte, nous n'avons trouvé des signes d'imprégnation calcaire fragmentaire que dans cinq cas, sur plusieurs milliers d'examens pratiquées”: he does not give an illustrated example.In our experience radiological examination of some thousands of larynges both during life and as post-mortem specimens has not shown evidence of calcification of the epiglottis. Recently, however, we have seen a patient in whom a portion of the epiglottis was well calcified. In view of its rarity it is considered that some observations on this patient may be of value.The patient was a man of 68 whose voice has “come and gone” for over ten years. When his larynx was examined in 1955 the vocal cords were noted to be reddened. His complaint has been a little worse recently so he was re-examined.

The incidence of calcification in 1,088 cases of breast disease and neoplasms in other sites was assessed radiologically and histologically.The examination of paraffin blocks of tissue revealed calcification in 42 per cent of carcinomas of the breast, with a lower incidence and different pattern in benign mammary lesions. This agreed closely with our experience based on mammograms in which 39 per cent of carcinomas showed calcification, indicating the quality of the clinical radiographs. Some possible factors in the pathogenesis of calcification in neoplasia are discussed.

TheYear Book of Radiologyfor 1964–65 is, like its predecessors, a mine of information. The editors are to be congratulated on their happy selection from the literature of that year and on the clarity of the abstracts.There are so many facets of radiology on which new work has been done that it is difficult for any one radiologist to keep abreast of all the recent advances, and this book may well be of help in elucidating some out-of-the-way problem. It should therefore be available in all major X-ray departments.
