摘要:
1. The Laruelle method of base-line ventriculography with small quantities of air, 5 c.c. to 10 c.c., is described. Owing to the small quantity of air used there is very little reaction, and encephalography can be done as an out-patient procedure.2. The absence of disturbance to the patient has made it possible to use this method in investigation of conditions such as epilepsy and cerebral trauma.3. This method should be the first line of investigation in any patient where encephalography or ventriculography may be indicated. This will demonstrate whether any useful information is likely to be obtained by encephalography with larger quantities of air, or ventriculography.4. It is not claimed that this method can replace in every case encephalography with larger quantities of air, or ventriculography.5. Cases of cerebral tumours, trauma, and epilepsy, are demonstrated to show the value of the method.6. The method is of value in the treatment of headaches of post-traumatic and indeterminate origin.

It is with some trepidation that I bring forward this short communication with the object of emphasising the use of a very simple position for radiography of the fourth ventricle.I cannot claim that the position is now a new one, as only last week a paper, which was read by Dr. Lysholm in June 1937, appeared in the BritishJournal ofRadiologyfor May 1938, and some of the illustrations show the position to which I am going to refer. Similar radiographic positions are also reproduced in a paper published in theAmerican Journal of Roentgenologyfor April 1938 by Dr. Robert Stone and Dr. Fritz J. Schulze. There are probably many others using it, but nevertheless I do not think that the importance of this posture is yet generally known. I say this because within the past year or two queries have been received from several sources asking for information as to the technique employed in radiography of the fourth ventricle.Working in collaboration with Mr. A. A. McConnell, F.R.C.S.I., we have used this position in the Richmond Hospital, Dublin, since early in 1935.It is not my intention to discuss ventriculography in general, but to confine my remarks to one aspect of this subject,i.e.,the necessity of adopting a technique which will enable one to visualise the fourth ventricle if it is not occluded and if the aqueduct of Sylvius is patent.

The notes of the following case, observed by me a few years ago, have not, as yet, been published; I had hoped to have been in a position to confirm or correct my impressions of it by the study of another case of like nature, either in my own practice or in the literature of the subject. As I have not been able to do so, and as the appearances are of considerable interest, I am now sending the notes for publication, and I should naturally be extremely grateful to any reader who might be kind enough to give me information of any similar case.George D —, a man of fifty-six years, was referred to me for a radiological investigation of the stomach on June 28, 1931. He had a history, extending over several years, of recurrent attacks of epigastric pain, bearing no definite relationship to food and not relieved by vomiting. The pain had completely subsided about eight months previously, but about six weeks before this examination the symptoms had returned in an aggravated form, and the pain was now practically constant, accompanied by acid eructations and occasional vomiting. The general state was poor; there was some epigastric tenderness, but no palpable tumour, resistance, or rigidity.The examination was extremely difficult, from various causes, and it must be confessed that it was not possible to obtain a satisfactory radiogram.

On Depth Doses from Teleradium Units |

摘要:
An experimental teleradium unit is described, in which it is arranged that focal “skin” distance, thickness of filter and area of source, may be varied. It is shown experimentally that the amount of soft radiation at the aperture increases with focal “skin” distance, as does the percentage depth dose. Results of experiments with varying thicknesses of lead filter show that the first 0·5 mm. of lead, together with 0·6 mm. brass, produce the maximum improvement in depth dose. Varying the area of the source produces little difference in depth dose for large focal “skin” distances, but for smaller ones (2–3 cm.) increases the depth dose by some 20 per cent.It appears that the inverse square law alone does not predict accurately the depth dose observed, and that an apparent absorption coefficient equal to 0·25 cm.−1is also required to bring the theory and experiment into agreement for heavily filtered sources.

摘要:
A number of physical problems which arise in the use of radium surface applicators of small area are considered from the point of view of radium and radiation distribution. Depth dose curves show the advantages of the use of larger radium skin distances, the improvement being particularly marked at the edge of the applicator. The effects of errors in radium distribution and radium skin distance are indicated. With a view to overcoming some of the difficulties, specifications of a number of standard small applicators are given.
