摘要:
The effect of a systematic protocol for oral care on the development of stomatitis in patients receiving chemotherapy was investigated. Using a systematic assessment with a four-point rating scale for each item, 114 evaluations of physical condition, oral condition and oral perceptions were done on a control group of 25 patients every other day for 25 days. A protocol for oral care was then instituted and 103 evaluations were done on 22 experimental patients. Findings indicate a significant improvement (p<0.01) in oral status when the oral care protocol was used. The physical condition of the groups did not differ significantly. Perceptions did not change significantly, although it was found that perceptions are a questionable indicator of oral status. This supports the necessity of regular physical assessment of the mouth. The level of infection decreased to two-thirds of the level in the control group when the protocol was utilized.

摘要:
The establishment of a nurse-run Chemotherapy Clinic in a Veterans Administration Medical Center is described. The reason for its founding, why a nurse was chosen and her qualifications, and the lines of responsibility within the hospital are outlined. The physical layout, day-to-day operation of the Clinic, and initial problems encountered are explained. The scope of practice of the nurse practitioner and the legal aspects of this new expanded role in the oncological setting are addressed. The author shares a view of her evolving philosophic stance for her practice, and its potential value in determining the thrust of cancer nursing in the future.

The major problems of psychosexual adjustment to total pelvic exenteration (TPE) were identified during interviews with patients who have undergone this aggressive cancer treatment. The “normal” psychosexual adjustment encompasses psychologic, social, and sexual factors. Based on these known influencing factors of adjustment, patients were questioned concerning their personal experiences in semistructured interviews. Specific problems involved such issues as body image, social life-style, and sexual relations and practices. Nurses have the professional responsibility of nurturing the adjustment of these individuals such that they redevelop their life to what they consider a respectable and worthwhile level. By specific interactions with the patient and her partner during the preoperative and postoperative periods, the nurse can promote the caring relationship necessary for optimum adjustment to this type of procedure. Awareness of the problems experienced by TPE patients and knowledge of how to deal with such problems are essential if the nurse as a permanent figure in the health care system is to effectively direct patients' toward an adjustment that fosters realistic goals and satisfying living.
