This article describes an exploratory, cross-sectional, correlational study designed to examine the relationships among general coping style, hope, and anticipatory grief in a convenience sample of 61 family members of people with terminal cancer. The framework for this study was based on grief theory, stress and coping, and transitions(2). Data were collected by the Jalowiec Coping Scale(3), the Herth Hope Index(4), the Non-Death Version of the Grief Experience Inventory(5), and a background information sheet. Findings revealed that family members experienced individual anticipatory grief patterns. Women reported more anger/hostility and despair than men. Adult children, more highly educated family members, family members not living with the ill person, and nonprimary caregivers also expressed more anger/hostility. Multiple regression results showed that emotive coping and hope accounted for significant variance in despair, somatization, and loss of control. Emotive coping contributed significant variation in anger/hostility, whereas lack of hope accounted for variation in social isolation. Neither the general coping styles nor hope significantly predicted death anxiety. The findings provide a base for future research and nursing practice.

The aim of this study was to evaluate the effect of nursing intervention on hope and quality of life in cancer patients. The sample consisted of 96 newly diagnosed Norwegian cancer patients ages 26 to 78 years, the majority of them women (71%). Breast cancer was the predominant type of cancer in this sample. The study used an experimental design in which patients were randomly allocated to three different groups. The first group was the experimental group, for which the intervention was designed to increase hope. The second group (attention control group) participated in the "Learning to Live with Cancer" program. The last group was a control group. The Nowotny Hope Scale was used to measure hope, and the Ferrans and Powers Quality of Life Index and the Cancer Rehabilitation and Evaluation Systems, short form, were used to measure quality of life. The questionnaires were completed four times: twice before, then 2 weeks and finally 6 months after the intervention. The level of hope was significantly increased (p= 0.020) for the members of the hope group just after the intervention, but not after 6 months. Despite the patients' positive evaluation of the intervention, there was no impact on quality of life.

The present study attempts to classify cancer patients' psychologic responses in order of importance, and to identify the role of the oncology nursing staff in the moral support of these patients. We want to emphasize the significant role of the nursing staff because until 1990, Greece had no specialization in the field of oncology nursing. After the initial diagnosis of cancer, the patient was considered as emotionally depressed.Our sample consisted of 120 cancer patients from the southwestern regions of Greece. From our research, we concluded that the factors generally characterizing cancer patients, in order of priority, are (a) moral support from family and friends (p< 0.0001), (b) psychologic reactions after some months (p< 0.0001), (c) sex (p= 0.0006), (d) age (p< 0.0001), (e) marital status (p< 0.0001), and (f) psychologic reactions during the first days following diagnosis (p= 0.018) because of the shock experienced.More specifically, we wish to stress the absence of the consulting role on the part of the nursing staff. This finding is worth the keen attention of consultants on health matters. Nurses have a great deal to contribute to the emotional care of cancer patients, and are puzzled about the reasons why they are not asked to participate in this important function.

The purpose of this study was to describe family dynamics of families with cancer on the basis of Barnhill's framework for healthy family systems.The sample consisted of families in which one member had cancer. Both the patients (n= 96) and their relatives (n= 96) participated in the study (n= 192). The data for the study were collected using the Family Dynamics Questionnaire and the Family Dynamics Measure.The results indicated that the cancer of a single family member did not impair family functioning, but that family dynamics were considered quite good. There were no statistically significant differences between cancer patients and relatives on any of the family dynamics dimensions. However, an examination of sociodemographic characteristics did reveal some differences. Older relatives reported more enmeshment and rigidity than did younger relatives, whereas the latter reported more role conflict than older relatives. Older patients reported more rigidity than younger patients. Relatives who were men reported more enmeshment than women, whereas women reported more role conflict. Relatives of two-member families reported more rigidity than relatives with a larger family. Patients who reported a serious illness in the family described more mutuality, better flexibility, and clearer communication than patients who did not report such an illness. Also, relatives who mentioned a serious illness reported more mutuality and flexibility.

Many oncology patients receive chemotherapy drugs that have the potential to induce oral mucositis. If mucositis is not prevented, patients will have to manage the problems associated with mucositis: pain, local infection, and decreased ability to take fluids or food. At the time of this writing, clinical approaches for mucositis management are variable and generally ineffective. The mouth care program, PRO-SELF©: Mouth Aware (PSMA), presented in this article, was found to be a significant component of a self-care program that may have reduced the incidence of chemotherapy-induced mucositis. The PSMA program has three dimensions: (a) didactic information, (b) development of self-care exercises (skills), and (c) supportive interactions with a nurse in the setting where the patients are receiving their treatment. This program focuses on decreasing the direct (i.e., incidence and severity of mucositis) and indirect morbidities of oral mucositis (i.e., number of local infections, level of discomfort/pain, and disruption in fluid and/or food intake). It provides the critical dimensions (i.e., specific information, self-care exercises, and nurse support) to promote the prevention of mucositis. The PSMA program is designed to provide patients with a definitive self-care repertoire to manage chemotherapy-induced mucositis in the home without the direct supervision of a health care provider.

摘要:
Caring for dying children and their families is one of the most difficult and challenging aspects of professional nursing. An analysis of cultural influences provides insight into the diverse worldviews held by people in Japan and the United States that affect nursing care of dying children. Selected cultural aspects that influence behaviors in Japanese and American cultures are reviewed in this article. Characteristics of support models for dying children and their families established in Western cultures are discussed. This cross-cultural review of prevalent sociocultural influences on the care of dying children and their families in Japan and the United States clearly demonstrates numerous contrasts and parallels.

According to the World Health Organization (WHO) guidelines, oral morphine is the first choice drug for treating moderate to severe cancer-related pain. The fear of the side effects caused by this drug and the scarce information about prevention and management of these effects are the main reasons for the underuse of morphine. The aim of this paper is to provide a review of the literature on the side effects most frequently present both in the titration phase and during chronic administration of oral morphine and to describe the appropriate treatment.
