A questionnaire entitled “Health Survey” was developed for the purpose of discovering life-style changes due to cancer, as perceived by a group of ambulatory cancer patients. The questionnaire contained 18 items related to physical changes, 12 items related to psychosocial changes, and five items that dealt with relationships to health personnel. The “Health Survey” was distributed and collected over a 6-month period in oncology outpatient units throughout northern Illinois. A total of 340 questionnaires were returned; the data analysis reported here is based on 323 questionnaires returned before the arbitrary cut-off date.The researchers had anticipated that with the diagnosis of cancer, there would have been significant physical, psychological, and social changes in the cancer patient's life-style. Results of this study indicate that there were three physical parameters and no psychological or social parameters that the majority (greater than 50%) of ambulatory cancer patients indicated changed with the diagnosis of cancer. The physical parameters that the respondents indicated had changed were: 1) level of physical activity, 2) sleeping habits, and 3) weight. The results do not support the assumption that a diagnosis of cancer always produces broad changes in life-style. Instead, it appeared for this sample, that selected physical variables were altered with the diagnosis of the disease. In addition, the results indicate differences in the communication patterns of the respondents with their physicians and nurses, as well as their level of satisfaction with these interactions.

Transition Services is a model of nursing practice designed to offer personalized services and continuity of care to patients and families living with the changing demands of progressive, deteriorating illness. The model conceptualizes the nurse functioning in two interrelated roles: a problem-focused support system for patient and family; and a communication link between the patient-family system and the larger health care provider-community system. Specific goals of practice are identified for each role, and these goals provide direction for the implementation of Transition Services. A specific case is used to illustrate the complexities of implementing activities to meet these goals in the real world of families living with cancer. Particular attention is given to the value of partnership as an effective means for delivering personalized care during the final period of the family's time together.

A frequently undetected problem in the long-term management of cancer patients is their low level of compliance with the therapeutic regimen. Nurses assume erroneously that cancer patients are highly compliant; there is, however, little evidence to support this assumption. Indeed, available evidence suggests the opposite.Unlike the acute care hospital setting where decisions are made and executed by staff, in long-term or ongoing care, cancer patients have the prime responsibility for implementing their therapeutic regimen. Clearly, compliance levels may determine the overall effectiveness of therapy for cancer patients, but little research has been conducted on compliance behavior among patients with cancer. This paper describes a framework around which nurses can create a climate for compliance behavior. By creating a climate for compliance, improved effectiveness of cancer therapy and the subsequent improved quality of life for individuals with cancer should be the achieved result.

A descriptive study of 52 nurses' perceptions of the pain suffered by patients with documented pain from terminal or metastatic cancer was conducted in a midwestern university hospital over a 33-week period. A questionnaire was used which asked the nurses to assess the pain of one specific patient for whom they were caring and then asked about their beliefs concerning the use of pain drugs for patients in general who have metastatic or terminal cancer. Analysis of the data revealed that 89% of the nurses believed that the patients were receiving adequate medication for pain control and yet 67 % assessed that the patients suffered moderate pain. The goal for narcotic administration for 58% of the nurses was to reduce pain rather than relieve it. Although addiction appeared not to be a conscious factor in these nurses' assessment process, the data seem to indicate that they often perceived moderate relief of pain as an appropriate goal.

Various stress reduction techniques have been advocated to assist patients in dealing with the stress of the cancer experience. One of the most popular stress reduction techniques is relaxation, a technique which may help cancer patients cope with pain and decrease the anxiety and side effects associated with various treatments. Due to increased patient interest and a need for further research in this area, nurses need to be knowledgeable about relaxation techniques. Current relaxation literature is reviewed and a plan for teaching relaxation techniques to patients is proposed.
