SummaryThe application of bone marrow transplantation in combination with high-dose chemotherapy is being investigated as a possible method of circumventing the toxic medullary effects of chemotherapy. The following paper explains the procedure that is now being studied at Duke University Medical Center where bone marrow harvesting is being used with highdose BCNU therapy. The method of obtaining and storing the bone marrow and the nursing care associated with the procedure and high-dose chemotherapy are explained.

SummaryABSTRACTIn this overview, the author documents the costs of cancer both to society and to individual patients and families. Costs are classified by economists as (1) direct costs—hospital and physician's charges; (2) indirect costs—lost earnings due to morbidity and mortality; (3) psychosocial costs—the effects of overspending for medical care, such as loss of lifesavings, forced sale of home, or family disintegration. The financing of health care is described.The author reviews studies which have examined the expenditures of individual cancer patients and families that were associated with the disease.Cost-effectiveness analysis is described as an effective method for evaluating the costs and outcomes of various treatment interventions. Health care providers and patients can mutually decide on specific treatment interactions, based on the patient's ability to pay. Thus, the patient can be spared financial devastation due to uncontrolled expenditures after he has entered a health care system.

SummaryABSTRACTThe purpose of this study was to determine the relationship between significant others and fear of disease recurrence in patients in cancer remission. A sample, consisting of 30 mastectomy patients in cancer remission, was administered a Fear of Recurrence Questionnaire and a Significant Other Interview instrument.Data analyses indicated a strong negative relationship between significant others and fear of recurrence. Those subjects reporting fewer significant others had higher fear of recurrence scores, whereas those subjects reporting more significant others had lower fear of recurrence scores. A strong negative relationship was also found between the number of significant others whom the subject identified as understanding her health concerns and fear of recurrence.Analysis of the demographic data showed that age, marital status, extent of disease, type of adjuvant treatment, time since surgery, and time since last treatment were not significantly related to fear of recurrence.Discussion centered on the crucial relationship between subjects' number of significant others and their threat of disease recurrence. Also discussed was the importance of mobilizing support systems as a means of lessening the threat of disease recurrence and maximizing subjects' potential for quality of life.

SummaryABSTRACTAlthough the nursing literature contains many references to the teaching and support functions of nurses, a review of the consumer and other health care professional literature suggests that the potential importance of nurses in these roles is not widely accepted. A sample of 139 postmastectomy patients were interviewed to find out patients' perception of the teaching and support they received from nurses during their illness. Only 20% identified nurses as a significant source of information and 25% reported nurses were a significant source of emotional support.

SummaryWomen undergoing adjuvant chemotherapy following mastectomy obviously have many information needs—as do their spouses and children. Many of these women are also subjects in clinical trials, while rarely understanding just what that means. Hospital staff associated with the National Surgical Adjuvant Breast Project (NSABP) at Mt. Sinai Hospital in Cleveland have developed an innovative and inexpensive program to meet these diverse information needs while reducing the flow of repetitious questions and helping the patient's family better understand the women's treatment and problems. A special information and social program was developed for a large group of patients on clinical trials and their families. Physicians, a nutritionist, and cosmetologist made brief presentations and responded to a wide range of questions—many detailed, some personal.Most of the questions dealt with side effects of chemotherapy and with breast reconstruction. Issues of sexuality also surfaced strongly as an area of concern and confusion. The social portion of the program provided opportunity to ask informal questions. Families, in particular, were often learning basic information about chemotherapy for the first time.This article provides a detailed description of the program and its outcomes based on staff perception and formal surveying of participating patients. Finally, several suggestions for conducting similar programs are offered in light of this program's experience.
