Cisplatin is one of the most active cancer treatment agents available. Unfortunately, however, cisplatin causes many untoward side effects. Nurses play a major role in administering cisplatin and in preventing and managing the adverse effects associated with this drug. In order to maximize the quality of life of patients undergoing cisplatin treatment, nurses need a thorough knowledge of its uses, administration, and side effects. This article is the second of a two-part series about cisplatin. Part I provided a review of the mechanism of action, current uses, and administration guidelines. Part II discusses the most common side effects of cisplatin and the appropriate nursing assessment and management of patients undergoing treatment with this agent. In addition, future directions for the use of cisplatin and the use of alternative agents will be discussed

Worldwide, squamous cell cervical cancer and intraepithelial lesions (SIL) are a major source of morbidity and mortality. Compared to women in general, women with human immunodeficiency syndromes (HIV) are at higher risk of developing SIL. With the HIV epidemic escalating among women, prevalence, morbidity, and mortality related to SIL are likely to increase unless adequate prevention and detection programs are mounted. Consequently, this article provides background information for the design of such programs, focusing on selected biochemical risk factors and natural history of SIL in women infected with HIV (HIV+) and women in general (HIV-). Current screening policies are described, and implications for nursing research and policy are discussed.

The incidence of genital human papillomavirus (HPV) has increased 10-fold in the past 5 years, and HPV has recently been linked with cervical carcinoma. This retrospective study compared risk behaviors and co-factors of HPV between two groups of women diagnosed with HPV. Women attending a Student Health Center were younger and more likely to have more sex partners, chlamydia, and other sexually transmitted disease infections than women attending private gynecologists. These women were older, and more likely to be former smokers and to have had gardnerella infections than the women students. Age at first sexual intercourse and duration of oral contraceptive use did not differ between the groups. Diagnostic reports of HPV were compared, and women attending gynecologists had significantly higher colposcopy impressions and cervical biopsies than the student group. Women attending private gynecologists were more likely to receive laser treatment on several genital sites, whereas students were more likely to receive trichloracetic acid or cryotherapy on their cervix only.

Bereavement, a well-established threat to health and work performance, is one of the most universal human responses that nurses experience. Because patient death is frequent, oncology nurses commonly experience bereavement; they may identify with the family, feel sad at the death, or feel awkward in responding to a death. After patients die, nurses manage bereavement tasks such as making sense of the death, managing mild to intense emotions, and realigning relationships. These tasks become more difficult when multiple deaths are encountered or when a conflict about the death occurs. Understanding theories, models, tasks, and factors influencing their bereavement may help nurses to facilitate their own grief and to reduce bereavement overload. The purpose of this article is to review knowledge and factors that influence nurses' bereavement and offer guidelines for managing grief. Information for this article was derived from several sources: (a) a review of existing literature; (b) clinical experience; and (c) analysis of personal inventories from over 300 nurses attending the authors' bereavement workshops.

Based on a lower reported incidence of breast cancer in the Arab population in Israel, nutritional levels were evaluated in a case-control study of 33 Jewish and 10 Arab patients with breast cancer all matched with a first-degree family-related control. Demographic, gynecologic, obstetric, anthropometric, genetically tumor-related, and nutritional data were ascertained through interviews, tests, and questionnaires. Food consumption, calories, protein, fat, and fiber intake, and anthropomorphic measures were calculated. The following results were obtained:1.Calories of food consumption were significantly higher in Jewish and Arab patients than in their controls.2.Jewish patients consumed significantly higher levels of monounsaturated fat; Arab patients consumed significantly higher levels of dietary fiber.3. Animal protein intake was elevated in patients of both ethnic origins as compared with controls.4. Vegetable fat and monounsaturated fatty acids were elevated in Arab patients as compared with Jewish patients.5. Body weight of both Arab and Jewish patients was not significantly higher when compared with their controls.6. Energy consumption and obesity were higher in breast cancer patients than in the controls. This supporting evidence suggests an association between obesity and breast cancer occurrence.

The chief goal of this exploratory study was to discover what a woman's own experience of her illness meant to her–how she thinks of herself as a woman and a person with a health problem. The study was exploratory in nature and designed to expand our conceptual thinking about health and illness and the delivery of health-care services. The study was qualitative, and the principal method was the semi-structured, in-depth, face-to-face interview. All of the women in the study had been diagnosed with primary breast cancer and were in a variety of stages of treatment. What emerged from the study were explanatory stories that women constructed to chronicle their illness experience and interpret it. The theme under discussion here is hair loss as symbolic of larger cultural beliefs and values. Recommendations for interventions with women who experience hair loss as a traumatic event are offered in the context of a deeper cultural understanding of hair loss that is a consequence of cancer care.

Although much is written about biomedical progress in the treatment of cancer, less attention is paid to personal struggles for self-affirmation when faced with the reality of recurrence. Through the use of the narrative as a technique for disclosing embedded meaning in the world of ordinary events, courage emerges as the distinguishing phenomenon of self-affirming potential. Three accounts of actual persons living the crisis of recurrence illustrate opportunities for professional caregivers to find enrichment through their encounters in practice.

Euthanasia is not a new concept. However, there is a growing trend to legalize voluntary active euthanasia. The purpose of this study was to explore oncology nurses' attitudes toward voluntary active euthanasia. The population consisted of 200 registered nurses who were members of the Oncology Nurses' Society and who resided in Illinois, Indiana, Iowa, and Missouri. I developed a questionnaire using a Likert-type scale to measure the attitudes. A one-way analysis of variance was used for data analysis. Relationships among religious beliefs, personal experience, educational preparation, and years of practice as an oncology nurse were investigated. Religious belief was the only variable that was significant in the formation of attitudes toward voluntary active euthanasia.
