The prevalence, utilization, and efficacy of home care services for children dying from cancer was examined. Information was obtained from a survey completed by 85 institutions in 47 states, 76% of the sample solicited. Home care services were offered by 85.9% of institutions; 43.8% of these administered their own programs and 56.2% relied upon community-based services. Institutions which provided their own services offered home care to a greater proportion of eligible patients, had a larger proportion of families accept this option, and had a smaller proportion of children return to the hospital to die than institutions which used community agencies. The most frequent problems with community agencies included inexperience with pediatric issues and procedures, inadequate pain management, and reluctance of families to work with unfamiliar staff. Institutionally administered programs were more likely to provide regularly scheduled home visits, care by pediatric oncology nurses, and bereavement follow-up. The most important factors inhibiting institutions from developing their own home care programs were anticipated problems with pain and other symptom management, uncertainty as to family competency to provide care, and concern regarding the effects of home care on patients and their families. Potential problems in providing home care services for children should be balanced against the expected benefits to the patient and family.

Nurses are subjected to occupational stress factors that can result in the syndrome of burnout. The problem addressed by this study was the evaluation of levels of burnout in oncology nurses and variables that related to its occurrence. A randomized sample of 152 members of the Oncology Nursing Society who returned the tools used to measure burnout was included in the study analysis. These tools were the Staff Burnout Scale for Health Professionals and the Yasko survey tool. The study found that oncology nurses sampled were not at any greater risk of burnout than other hospital-based nurses. The oncology nurses in this study experienced a moderate degree of burnout, scoring a mean of 52.7 points on the Staff Burnout Scale. Potential range for scores was 20–140. This is only slightly higher than the mean of 51.1 reported by Joyce Yasko for clinical nurse specialists. Variables correlated with development of higher scores of burnout were availability of psychological support; amount of job stress or tension experienced; level of job satisfaction, i.e., the presence of positive or negative feelings about work; and the presence of certain sources of stress, such as organizational problems or physicians. Age and years of experience were mildly correlated with burnout scores.

Promoting a nurturing work environment is the focus of this paper. The work setting is an oncology unit, but the concept is universal to any health care situation. The nurturing roles and responsibilities of head nurse, clinical nurse specialist, staff nurses, and non-nursing colleagues are explored. Components of a nurturing environment are discussed, including the physical environment, multidisciplinary care teams, staff support mechanisms, quality assurance and research programs, and patient/family considerations. Open, nurturing communication (among nursing leaders and nurses) is emphasized as basic to job satisfaction and professional growth.

Hypercalcemia is a fairly common complication of certain types of malignancy. The manifestations of cancer-induced hypercalcemia may be unpleasant for the patient and are potentially life-threatening. Prompt diagnosis of this complication, followed by effective treatment, usually can reverse the condition. This article provides current information regarding incidence, pathophysiology, manifestations, treatment, and suggestions for possible prevention of symptoms of cancer-induced hypercalcemia. Nurses involved in the care of cancer patients need to be able to assess their patients for manifestations of hypercalcemia. Nurses also have a responsibility to make patients at risk to develop cancer-induced hypercalcemia aware of this complication and of the measures they may take to possibly prevent its occurrence.

This article explores the psychological concept of hope as it applies to patients with cancer and their families. Hope is an essential element in the lives of those with cancer, because it enables the living to continue on and the dying to die better. Nurses are in a unique position for enabling hope within the patient and family unit, due to the extensive contact they have with patients during the cumulative hospitalizations and outpatient care involved in cancer treatment. The role of oncology nurses in enabling realistic hope is discussed, and a number of nursing approaches toward this end are described. Examples of immediate, short-term, and long-term goals are presented. The article further explains how nurses become catalysts as they help the patient and family unit plan goals together.

The possible hazards associated with the occupational exposure to antineoplastic drugs have been acknowledged widely in recent years. Hospitals, several drug companies, and professional oncology organizations have responded by developing detailed guidelines and procedures for maximum protection for the worker involved. Standards for mixing, administering, handling, and disposing of these cytotoxic agents are the result of numerous research studies and their subsequent findings. Only a small percentage of chemotherapy, however, is given on an inpatient basis, or can be prepared for administration by a pharmacist in a protected area of a hospital pharmacy. In the oncologist's office, one or two nurses often must shoulder the responsibility of assisting with patients and securing and analyzing blood tests, as well as admixing and administering chemotherapy treatments. While guidelines approved for hospital and institutional use afford the worker optimal protection, they may be impractical and costly in a small oncology office. Specific steps can be taken regarding safe disposal of toxic waste, as well as safe techniques for mixing and administering chemotherapy, while maintaining a cost-conscious approach. Office nurses are encouraged to secure safety measures for their protection, including the acquisition of a vertical flow hood for optimal chemotherapy preparation.
