摘要:
A double‐blind parallel trial of astemizole, pheniramine and placebo was carried out in 15 patients with hay fever. Astemizole is a new potent H1‐antihistamine with long duration of action but devoid of central activity. Evaluation criteria were daily symptom‐ and side effect score cards, daily nasal peak flow measurements, clinical examination and blood tests. Control of nasal symptoms was significantly better with both antihistamines than with placebo. Tiredness and anticholinergic side effects were less common in the astemizole group than in the pheniramine group. No significant changes in the “safety evaluation” blood tests were

摘要:
A study was made of the influence of calcium antagonists on human basophil histamine release inducedin vitroby specific antigen, anti‐IgE or the calcium ionophore A23187. Both verapamil, nifedipine, and nimodipine were found to inhibit the release, and a similar effect was also observed after peroral administration of verapamil and nifedipine. The inhibitory effect of the drugs on histamine release seems to depend on interaction with calcium at different sites. PVG tats given injections of 1 μg ovalbumin (OA) together with 10 mg Silica gel failed lo provide serosal mast cells or lung tissue with the capacity to release histamine onin vitrochallenge with the antigen. However, if such animals were injected i.p. with 100 mg of alum (without any further antigen addition) 3–9 weeks after the primary antigen injection, their mast cells and lung tissue showed a clear‐cut capacity to respondin vitro, when examined 1 week after the alum injection. Injection of only 15 mg alum did not induce such a response capacity. The lading of the reactivity induced by an alum injection could be prevented by a repeated injection of the adjuvant alone. Pretreatment of the rats with cyclophosphamide (33 mg/kg) 2 days before the primary antigen injection did not affect the response capacity induced by a booster injection 3 weeks later. S.c. injection of alum also precipitated response capacity in animals primed by i.p. injection of antigen and Silica gel. The anaphylactic response capacity induced by injection(s) of alum was generally accompanied by increased levels of OA‐IgE and especially OA‐IgG2aantibody; however, a clear‐cut correlation between either serosal mast cell or lung tissue response capacity and serum OA‐IgE or IgG2aantibody titer could not be demonstrated. SD rats were sensitized by an i.p. injection of 1 μg or 10 μg ovalbumin (OA) together with 10 mg Silica gel. At the indicated time alter sensitization, allergic response capacity of the animals was estimated by measuring changes in intratracheal pressure induced by an i.v. injection of 0.3 mg OA (low challenge dose) or 5 mg OA (high challenge dose). Animals given 1 μg OA in Silica gel showed no response capacity at 14, 35 or 47–48 days after sensitization. Animals injected with 10 μg OA showed a clear‐cut response with a peak at 14 days; at 7 weeks after immunization the response capacity had faded almost completely. Specific OA‐IgE antibody and total IgE serum levels were examined by radioimmunoassay, A slight increase in OA‐IgE antibody was recorded in animals in injected 14 days before test with 10 μg ovalbumin and Silica gel; animals given 1 μg OA and Silica showed no OA‐IgE antibody.Five weeks after sensitization, some animals of each group were injected i.p. with 100 mg alum,B. pertussisvaccine (2 ml of Perthydral®), or saline, all injections being made without any further antigen addition. 12–13 days after such an injection, alum‐treated animals showed high response capacity at bronchial anaphylatic tests and high OA‐IgE antibody levels but comparably low levels of total IgE. Animals injected withB. pertussis, on the other hand, showed high response capacity at bronchial anaphylactic tests and OA‐IgE antibody levels) but high total IgE‐antibody levels. The bronchial anaphylactic response in theB. Pertussis‐injected animals but not the alum‐injected animals was significantly correlated to serum IgG2aantibody levels.These results show that injection of alum orB. pertussisvaccine without antigen can precipitate/enhance anaphylactic response capacity and production of specific

Leukocyte suspensions containing basophils were obtained from 23 patients with rheumatoid arthritis (RA). When these cells were incubated with leukocyte nuclei from normal persons, histamine release was seen in 11 of the 15 patients with active disease, but not in the quiescent group or in normal individuals. The dose‐response cure for histamine release was similar to that obtained by specific antigen in type I allergy. By removal from and refixation to the cells of surface Ig, the release of histamine was respectively, abolished and restored, just as in similar experiments in hay lever patients. The dependence of pH for removal was also identical with that found in type 1 allergy. Antinuclear antibodies of the IgE class (IgE ANA) mainly directed against the granulocyte nuclei were often found in serum and on the cell surface of the RA patients, but not in normal individuals. A correlation was found between these titres in serum and on the cell surface on the correlation was found between ANA in serum and on the cell surface, on the one hand and disease activity and histamine release on the other. In a group of 12 patients with another Joint disease, osteoarthrosis, only two patients showed histamine release, and in contrast to the other patients they showed swelling of more than two joints.
