Dietary intakes and biochemical markers of selected minerals: comparison of highly trained runners and untrained women. |

Mineral status of highly trained (HT) women runners (n = 14) and untrained (UT) women (n = 11), as determined from 3-day dietary records, blood, and urine samples, was compared. HT women had significantly higher energy (HT: 2331 +/−121 vs UT: 1973 +/−107 kcal/day; p less than 0.05), magnesium (Mg) (HT: 369 +/−29 vs UT: 262 +/−18 mg/day, p less than 0.05), and potassium (K) (HT: 3874 +/−384 vs UT: 2646 +/−244 mg/day; p less than 0.05) intakes than UT women. However, mean daily intakes of calcium (Ca), iron (Fe), copper (Cu), and sodium (Na) were not different for the two groups. Plasma concentrations and urinary excretion of Ca, Mg, and Cu of HT and UT women did not differ. Plasma Fe concentrations of the two groups were also similar (HT: 21 +/−2 vs UT: 22 +/−1 mumol/L), and, although serum ferritin concentrations of HT women were lower than UT women (HT: 18 +/−4 vs UT: 30 +/−6 micrograms/L), the difference was not significant. Urinary Na excretion of the two groups did not differ. HT women had a higher K intake than UT women but urinary K excretion of the two groups did not differ. Whether present mineral intakes by HT women are adequate during periods of intensive training remains to be determined.

A prospective study was performed in clinically malnourished patients in which liver function was tested during a 4-week period of total parenteral nutrition (TPN). The purpose was to determine if concomitant intravenous lipid administration would reduce liver function abnormalities noted to occur frequently in patients receiving TPN. Twenty-five patients were randomly assigned to receive either daily infusions of 200 cc of a 20% lipid emulsion with TPN or TPN without lipid for the first week. In the subsequent 3 weeks all patients received daily intravenous lipid. The early lipid treatment group received 0.7 g lipid/kg BW/day and approximately 280 mg of choline/day from the lecithin emulsifier throughout the entire study period. Liver function tests were performed twice in the first week, then weekly thereafter. There were significant (p less than 0.05) elevations in liver function tests in the early lipid treatment group (for aspartate aminotransferase in weeks 1, 2, and 3, and lactic acid dehydrogenase in weeks 2 and 3). Alkaline phosphatase activity was elevated at weeks 2, 3, and 4 for the lipid-treatment group and at week 1 for the lipid-restricted group. The two groups had a similar elevation in gamma-glutamyltransferase activity. Analysis of covariance demonstrated that the overall duration of TPN, and not the presence or absence of intravenous lipid, was significantly related to the elevations in both alkaline phosphatase and gamma-glutamyltransferase (GGT) levels. In contrast, the early intravenous administration of lipid was significantly related to the increase in aspartate aminotransferase levels. The peak increase in AST was noted at day 7 in the lipid-administration group.(ABSTRACT TRUNCATED AT 250 WORDS)

Six patients with acute head injury (initial GCS 4.8 +/−1.7) were studied to determine the contribution of protein oxidation to resting energy expenditure (REE). Patients were studied on the second or third day post-injury and prior to implementation of nutritional support. Variables measured included REE by indirect calorimetry (normalized to percent predicted energy expenditure calculated from the Harris-Benedict equation). 24-hr urinary nitrogen excretion, calorie, and nitrogen intake. All patients received dexamethasone (39 +/−2 mg/day) and three received pentobarbital. Mean REE was widely variable, ranging from 43 to 128% of predicted (mean, 90 +/−31%). Mean 24-hr urinary nitrogen excretion was 16.5 +/−5.8 g. The contribution of protein oxidation to REE was 30 +/−4%. The contribution of protein oxidation to REE did not parallel REE (r = -0.237, p = NS) or REE expressed as percent predicted (r = -0.258, p = NS). The contribution of protein oxidation to energy expenditure is greater in acute heat trauma than previously described soft tissue injury and sepsis. The observed excessive nitrogen catabolism and increased contribution of protein oxidation to resting energy expenditure suggest accentuated protein requirements in respect to energy needs in head-injured patients.
