SummaryTwelve patients with primary hyperlipoproteinemia type IIaaccording to Fredrickson were treated after a six week placebo period with a new cholesterol-lowering resin. Colestipol (U-26.597 A, Upjohn). The drug was given orally, five g three times daily. Ten patients completed the trial and were treated with Colestipol during one year. From the third month of treatment a significant decrease of their cholesterolemia was observed (P<0.05). This cholesterollowering effect persisted after twelve months with a mean decrease at the end of the trial of 16.6%. Serum triglycerides, glycemia and parameters of the calcium and phosphate metabolism were not influenced by the resin. A decrease in uricemia was noticed. The two patients with an abnormal oral Glucose Tolerance Test responded less or not at all. One patient interrupted the treatment due to gastro-intestinal complaints. The other patients did not show any untoward reactions except for episodes of moderate constipation. There were no signs of toxicity.

SummaryThe authors present clinical, anatomopathological and immunological documents showing that tuberculosis is still active in our countries, that it often goes unnoticed and is only diagnosed in the autopsy room, that it contaminates persons without apparent debilitating disease.In the acute and subacute forms, they suggest, after sampling and before bacteriological confirmation, a thera-peutical trial, associating the bactericidal actions of Isonia-zide and Ethambutol during a maximum of 6 weeks, followed, if successful and if K.B. has been discovered, by the threefold therapy Isoniazid–Rifampicin–Ethambutol.

AbstractThe authors present 31 cases of gas gangrene which they define as a (sub)acute necrosis of muscles and/or subcutaneous cell tissue due to gas-producing organisms. This series includes 20 clostridial and 11 non clostridial gas gangrene cases.In the group of 20 clostridial gas gangrene cases, infection developed mostly either in a traumatic wound of the soft tissues (in some cases with acute ischemia due to a vascular lesion) or of the stump after amputation forobliterative arteriopathy. The clinical picture showed an extensive local lesion (discolored or streaked skin, foul-smelling serohematic exudate, necrotic muscles etc.) associated with often severe signs of systemic toxemia. From the bacteriological viewpoint, direct examination of a smear from the wound showed the presence of Gram-positive bacilli suggestive of Clostridium in almost all patients. In 18 cases, anaerobic culture confirmed the presence of such microorganisms (C. welchii). Treatment included general resuscitation procedures in association with high doses of penicillin, most conservative possible surgical procedures, and use of hyperbaric oxygen. Mortality throughout hospitalization of this group of 20 patients with gas gangrene was 30 percent (6 deaths). Eight patients were cured with practically no sequelae while the other six had more considerable sequelae.Non-clostridial gas gangrene cases (11) were due to a number of causes such as various surgical procedures, traumatisms of the limbs, and neoplasms. The wounds were erythematous in appearance, with muscle necrosis in half the cases; there was a fetid purulent exudate. In some instances, the general signs of infection were severe (septic shock in 3 cases). Bacteriological examination revealed aerobic microorganisms such as E. coli, Klebsiella, or Proteus in all patients, associated with anaerobic organisms in 4 cases (Bac-teroides). Treatment included surgical procedures (simple drainage of purulent collections in most cases). Hyperbaric oxygen therapy was systematically initiated, but discontinued as soon as absence of Clostridium species was confirmed. When smears from the wound revealed Gram-negative organisms (coliforms and/or Bacteroides), it seemed reasonable to initiate an antibiotic therapy using clindamycin and gentamicin. In this group of 11 cases of non clostridial gas gangrene, mortality rate was 18 percent (2 deaths). Seven patients were cured with practically no sequelae.The basic elements of the differential diagnosis of both clostridial and non clostridial gas gangrenes are reviewed. notably with respect to incubation time and the speed with which the wound tends to expand, wound appearance and characteristics of exudate.

SummaryA fair correspondence was found between the label and assessed factor-VIII and -IX of the Immuno concentrates Kryobulin (factor-VIII) and Bebulin (factor-IX).The mean in vivo recovery after 42 infusions of Kryobulin in 26 haemophilia A patients was 143%; after 8 infusions of a non-lyophilized cryoprecipitate in 8 haemophilia A patients 116 %. The observed in vivo factor-VIII gain per unit of concentrated factor-VIII (Kryobulin) administered per kg body weight was 2.03. The metabolic half-disappearance time of factor-VIII when administered as Kryobulin or a lyophilized preparation prepared by the Belgian Red Cross was 8.5 and 9 hours respectively.In total 18 infusions of Bebulin have been given to 7 haemophilia B patients. Based on label activity, the mean initial dose infused was 30.71 U per kg. The mean factor-IX rise immediately after the end of perfusion was only 15.7% whereas the expected increase was 74.8%. The mean in vivo gain administered per unit of factor-IX (Bebulin) was 0.41 (0.11-0.76). The half-disappearance time of factor-IX was circa 13 hours after Bebulin and 15 hours after plasma administration.Satisfactory haemostasis was obtained with Kryobulin (42 infusions) and Bebulin (18 infusions). Out of 3 haemophiliacs treated with one of the two Immuno concentrates 3 developed overt hepatitis. As other blood products were also administered, causal relationship with the Immuno products remains uncertain. No other major side effects were noted.The advantages of the lyophilized Immuno concentrate Kryobulin over fresh or deep frozen cryoprecipitate are its pre-assayed potency, small volume, virtual absence of isoagglutinin and a reasonable solubility. The same advantages hold when Bebulin is compared to plasma.
