Summary1. Sarcoidosis is characterized by granuloma formation in many tissues. The origin of the granuloma formation remains unknown.2. The Kveim reaction (intradermic injection of a suspension prepared from sarcoid tissue) is a reliable aid in the diagnosis of sarcoidosis when a validated suspension is used. The reaction seems to be the expression of a specific lymphocyte response to an antigen of the suspension.3. The number of circulating lymphocytes is decreased in the active forms.4. Many parameters of T-cell function point to a decreased cellular immunity (impaired delayed hypersensitivity skin reactions, low E-rosette, impaired antigenic and mitogenic stimulation).5. Antilymphocytic factors can be demonstrated in sarcoidosis serum and are perhaps responsible for the depression of T-cell function.6. Other findings suggest a concomittant T-cell stimulation (atypical lymphocytes, increased spontaneous DNA formation and MIF production, Kveim reaction as an expression of cellular immunity).7. The level of immunoglobins is significantly increased. There is a raised antibody titer against several viruses. This is interpreted as a stimulation of the humoral system.8. Circulating immunecomplexes have been demonstrated in a limited number of cases.9. The possible role of lymphocyte interactions and feedback mechanisms in the phenomena observed is discussed.

SummaryThe authors report six cases of bloody left sided pleural effusion in relation with a subdiaphragmatic lesion. In the first three cases they discover a pancreatic pseudocyst which fistulized into the pleural cavity and produced a very high level of amylase activity. The diagnostic must be proved by the early injection of ultra-fluid contrast medium into the pleural cavity. The three other cases were: one haemorrhagie adenoma of the left adrenal gland, one intrasplenic reticulosarcoma and one intraabdominal reticulosarcoma. SummaryNine years after thymectomy, efficient on the neurologic symptoms of Myasthenia Gravis, a 29 years old female developped systemic Lupus Erythematosus. There was no exacerbation of Myasthenia Gravis and immunological investigations showed a T lymphocyte defect. This is the fourth reported case of systemic Lupus Erythematosus after adult thymectomy for Myasthenia Gravis. The influence of adult thymectomy on the immune competence is reviewed. The possible role of this operation on the appearance of the lupus syndrome is discussed.

SummaryA white male patient 38 years old has contracted leprosy in Africa. After several years of incubation, dermatological manifestations occured (erythemato-squamous patches mainly located on the trunk) together with neurological signs (distal anesthesia of lower limbs going up to the thighs and localized zone of thermoanalgesia around the right forearm). Histological and immunological findings enabled to classify the disease within the«border-line»group, with a lepromatous tendency. A review of recent data concerning leprous neuritis emphasizes its close relationship with the immunological response of the host. The therapeutic prescriptions resulting from this relationship are described.

AbstractThe therapeutical benefits ofβ-receptor blocking agents, particularly of Propranolol, in clinical neurology are discussed. The authors report their experience with Propranolol in the treatment of essential tremor and of non-resting tremor in Parkinson’s disease. 45 patients (mean age: 52) with essential tremor reduccd by alcohol were treated with Propranolol 40 mgr t.d. in an open study. Accelerometric and electromyographic recordings were made before and during treatment. 10 patients showed dramatic improvement after Propranolol, 27 had definite therapeutical benefits; in 8 patients the drug was uneffective initially but 4 of them responded well at a second trial. In clinically improved patients, the amplitude of the tremor was reduced, but not its frequency. Adverse effects were neglectible. No therapeutic exhaustion was evident after long term therapy. Pindolol and Practolol had no effect on essential tremor. In 15 patients with Parkinson’s disease and non-resting tremor. Propranolol was compared with classical antiparkinsonian drugs. Propranolol was effective in 77 % of these patients in reducing the disabling non-resting tremor; occasionally this drug was successfully combined with L-Dopa or Amantadine.From the author’s experience and from most reports in the literature it can be concluded that Propranolol is the most effective drug on essential tremor. In addition,β-receptor blocking agents reduce the tremor observed in thyrotoxicosis, anxiety states and alcoholic withdrawal syndromes. They have given promising results as a prophylactic treatment of migraine attacks. The exact mechanisms by which these drugs are effective in neurological disorders remain controversial. It is now well established that Propranolol acts centrally on essential tremor. It has been suggested that this central effect is due to its membrane stabilizing properties. However, this hypothesis remains to be confirmed.
