SummaryAt regular intervals, new strains of type A influenza virus, immunologically distinct from the previous ones, strike unprotected population.This year, a new strain appeared, the Far-East strain, which rapidly invaded the whole world.After a general survey of influenza, we are presenting our experience with the type A/Singapour/1/57 virus.1. The inoculation of material obtained at the beginning of the disease permitted the isolation of the virus in 12 cases out of 14. Between the 5th and 10th days of illness, the isolation was successful in only 1 case out of 4.Several successive fatal cases were observed. In a case of influenzal encephalitis the virus was isolated from autopsy material. This could indicate the acquisition of neurotropic properties by the influenza virus.2. The evolution of the antibody titers was followed by the complement-fixation and hemagglutination inhibition tests. A significant rise in the antibody levels was generally noted in convalescent individuals. I.H. antibodies attained a mean titer of 320 U/ml between the second and third week from the beginning of the disease. This titer is lowered to 80 U/ml after the tenth week.3. An experimental vaccine was prepared on chicken embryos. The virus was precipitated by the addition of protamine sulfate. The virus-sulfate mixture was washed several times, suspended in a phosphate-buffered solution and finally inactivated by formol. The titer of this vaccine was 320 U.H./ml, and its nitrogen content 50 mg/liter.4. When it was administered to 1,000 individuals, the vaccine caused mild allergic reactions in 5 % of cases. Determination of antibody levels were made 8 or 9 days after vaccination. By mixing different dilutions of serum with different dilutions of virus, it was shown that the complement fixators were parallel to the neutralizing antibodies.These mixtures were inoculated in the allantoid cavity of chick embryos. The serum of vaccinated subjects gave a protection as good as the protection given by convalescent serum.

SummaryDuring a recent epidemy of influenza, 6 cases of serious pulmonary complications were observed, including one death. As has been previously observed, the severity of the influenza is conditioned by the secondary pneumopathy. The staphylococcal complication is the most severe for it causes serious toxic reactions and very extensive pulmonary changes, which usually respond to antibiotic therapy.The pathologic, clinical and X-ray findings of these pulmonary complications are again described. The factors influencing the incidence of bacterial pulmonary complications in the course of influenza are discussed.

SummaryThe clinical evolution of our cases, the X-ray findings and the agglutination titer of the Hirst reaction suggest the diagnosis of viral pneumonia due to Singapore influenza, although a secondary bacterial infection should not be excluded.One of our patients suffered a bacterial surinfection due to Staphylococcus aureus. This complication is extremely frequent and its importance has been stressed by Dutch workers (Mulder, Lankaster, Beeuwkes).

SummaryA fatal case of influenza is described. The 57 Singapour virus could be isolated. Post-mortem examination showed extensive bilateral bronchopneumonia, with bronchiolitis, bronchitis and ulcerative tracheitis. Cultures of autopsy material and bronchial secretions yielded a Staphylococcus aureus strain which was sensitive in vitro to currently used antibiotics. There were also important pleural effusions which remained sterile on culture.The fulminating course of the disease did not permit any recording of original laboratory data and the case seems quite common when it is properly situated in the literature. Nevertheless, it was thought that it could be interesting to compare it with those published by others.The therapeutic management of such cases is discussed.

SummaryA case of acute influenzal meningo-encephalitis is described. Its influenzal origin was demonstrated serologically. The psychic symptomatology was very peculiar and could be divided into three stages : ambulatory automatism, excito-subconfusion, maniac episode. The neurologic and meningeal signs are discrete and transient. The clinical evolution suggests a cerebral lesion directly due to the neurotropic influenza virus. The psychic symptomatology, the fugacity of the meningeal and neurologic signs and the spontaneous cure without sequellae depict the benignity of the case.

SummaryAmong 57 patients showing ischemic syndrome of the leg, the trouble was due to dilated varicose veins in 7 cases.It is thought that the ischemic disturbance is due to transient arterial spasms secondary to veno-arterial reflexes arising from baro-sensitive nervous endings in the venous wall.Such a reflex could be experimentally reproduced by venous stasis with oscillographic recording of the arterial spasms.The ischemic syndrome of the leg can arise from dilatation of varicose veins, eventually associated with primary arterial factors.

Summary1. In a series of 659 patients followed up from one to ten years after a subtotal gastrectomy for ulcer, cancer, tumor of connective tissue or polyp, an anemia (Hb<12 g/100 ml) appeared in 17 % of the cases. The mean hemoglobin values were 14,1 g for men, 12,5 g for women, or about 2 g lower than the values observed in a normal population. The anemia was hypochromic in 15 % of the cases, megaloblastic in 2 %.2. Hypochromic anemias are due to several factors :a) The role of gastric achlorhydria is of little importance : 135 achlorhydric patients had a mean hemoglobin value of 13,1 g, 23 % were anemic; 74 chlorhydric patients had a mean hemoglobin value of 13,7 g : 14 % were anemic.b) A postoperative protein deficiency reduces simultaneously the hemoglobin and serum albumin levels. In 122 cases studied, a positive correlation was observed between hemoglobin and serum protein and albumin values.c) The preoperative hemoglobin level. Patients predisposed to anemia before gastric resection keep the same tendency after the operation.d) The postoperative hemoglobin level. Anemia is much more frequent in patients with a low postoperative hemoglobin. After subtotal gastrectomy factors a-c interfere with the normal hemoglobin synthesis. It is therefore important that all blood deficiencies occuring before and during surgery be perfectly corrected.3. The few cases (14) of megaloblastic anemia encountered in partially gastrectomized patients are not a consequence of the gastric resection. Most of them are seen in patients operated on for cancer (6 cases or 4,1 %) or polyp (7 cases or 35,0 %) : in these cases, the disease is already present before the gastrectomy. Some very rare cases (1 or 0,2 %) appear after resection for peptic ulcer; their frequency being identical to the percentages observed in a normal population, it must be concluded that their appearance is a pure coincidence.

SummaryThe authors describe a case of metastatic carcinoid tumour. The clinical diagnosis was made upon finding typical symptoms and signs :«flushing», diarrhea, hepatomegaly.Histological examination and the presence of great quantities of 5-hydroxy-indolacetic acid in the urine confirmed the diagnosis. The clinical and physiopathological aspects of the disease are discussed.
