SummaryIn a double blind, randomized study, the effects of powder inhalation of 40figversus 400figof ipratropium bromide were compared in ten patients with obstructive lung disease in whom 2 X 20figof the commercial dose-aerosol (Atrovent®) was ineffective (mean change in FEV, after 15 min: + 4, 7 %).Both doses of powder inhalation caused a clearcut bronchodilation which generally reached a plateau between 30 and 180 min. 400figwas slightly more effective than 40figalthough the differences were statistically significant only at some time points: after 30 min the change in FEV, was 26±13 (SD) % versus 16±11 % (p<0.05).There were no appreciable side effects even not with the high dose.

SummaryThis case report concerns a 44 year old man presenting with a systolic murmur on the back of the thorax, an elevated systolic blood pressure in the upper extremities, differential blood pressure between upper and lower extremities and decreased pulses in the lower extremities. Computed tomography of the thorax, after contrast injection, revealed extensive calcification with narrowing at the descending thoracic aorta. Aorta catheterization detected a pressure gradient of 20 mm Hg over this atherosclerotic narrowing, localized at the descending aorta. Aortic angiography revealed an irregular narrowing of the descending aorta and also a se cond irregular narrowing, immediately under the origin of the cranial mesenteric artery extending over two vertebras.A review of the literature concerning coarctation and pseudocoarctation of the aorta allows us to conclude that the presented case is a variant form of aortic coarctation (not described until now). This variant form is due neither to a congenital anomaly nor to kinking of the aorta but to atherosclerosis.

SummaryElectrocardiographic abnormalities have long been reported in patients with central nervous system disorders.Both enzymatic and microscopical evidence confirmed the presence of organic myocardial injury.Experimentally, sympathetic and parasympathetic stimulation as well as catecholamine infusion produced myocardial lesions similar to those observed in subarachnoid hemorrhage.The efficacy of beta-blockers in preventing myocardial damage has been proved.Wc report a case of subarachnoid hemorrhage simulating acute transmural myocardial infarction.

SummaryWe are reporting the case of a 55-year-old man with stage II chronic lymphoid leukemia, complicated by Herpes zoster ophtalmicus with contralateral hemiplegia. This hemiplegia has been interpreted as the consequence of a granulomatous angiitis. Anti-viral drugs should be useful in the treatment of these cases.

SummaryThe clinical and bacteriological observation of a 52 year old man with pneumococcal endocarditis is presented. Two successive surgical interventions combined with appropriate antibiotic treatment cured our patient. The pathogenesis of pneumococcal infection and the problems involved in bacteriological diagnosis are discussed.

SummaryA case of fever in a 55-year-old woman is presented. Symptoms, vague swelling and warmth of both legs, started thirteen years after an aortofemoral prothesis.Aortoenteric fistula due to an infected vascular graft was diagnosed at the time of a massive gastrointestinal haemorrhage, 6 months after the onset of symptoms.The patient died during surgical intervention.

SummarySodium fluoride, calcium and cholecalciferol were given to a patient, suffering from a postmenopausal age related osteoporosis. The treatment led to severe hypercalcemia and acute renal failure. Clinical investigations disclosed enventually the presence of sarcoidosis.We suggest that the use of vitamin D in the treatment of osteoporosis requires a careful monitoring of blood calcium and/or calciuria.
