SummaryLymphoproliferati ve diseases may occur as a complication of severe immunosuppression. In our own center two out of 46 heart transplant recipients, receiving a cyclosporin A containing immunosuppressive regimen, presented with this complication and are reported here. Hepatic involvement and an occult small bowel localisation, which only became evident after a chemotherapy-induced small bowel perforation, illustrate the predilection of posttrans-plant lymphoproliferations for extranodal sites. Both cases represent the extremes of a spectrum of post-transplant lymphoproliferations ranging from polyclonal benign lesions to monoclonal malignant lymphomas. Their B-cell origin was pathologically established, but in none of the two repotted cases the previously postulated role of Epstein-Barr virus in the pathogenesis could be confirmed by serological tests. Severe immunosuppression played a major role in the pathogenesis of these disorders, as suggested by concomitant opportunistic infections occurring in both cases.The lymphoproliferation diagnosed in the polyclonal stage was reversible after reduction of the immunosuppressive therapy, without subsequent graft

SummaryIn 9 out of 14 patients with chronic eosinophilic pneumonia (CEP), followed-up in our department, circulating immune complexes (CIC) and other immunologic indices were investigated. The diagnosis of CEP was based on the typical clinical and radiographic findings and on the demonstration of eosinophilia in serum, bronchoalveolar lavage (B AL), fiberoptic transbronchial biopsies and surgical lung biopsies. CIC measured by solid-phase monoclonal RF binding were always strongly positive during a flare-up of CEP, and CIC measured by solid-phase Ciqbinding were also positive in most cases. Remissions of CEP were accompanied by lowering or disappearance of CIC. CIC were significantly correlated with eosinophil ia (p<0.001 for RF and p<0.02 for Ciq). Other immunologic indices were normal except for increased IgE in two thirds of the patients, and a slightly positive rheumafactor without rheumatic disease in half of the patients.The CIC are thus related to disease activity, and can be used as one of the markers of it, in addition to the classical markers such as eosinophilia and chest X-ray. However, this does not imply that the CIC have a pathogenetic role in CEP; these CIC can, indeed, be an epiphenomenon only.

SummaryTwo patients are described with acute streptococcal myositis. One of them died after a brief duration of illness in multiple organ failure; the other survived extensive muscular damage complicated by diffuse intravascular coagulation, acute renal failure, adult respiratory distress syndrome, bronchopneumonia, Pseudomonas septicaemia and probably streptococcal toxic shock syndrome. Both patients received nonsteroidal antiphlogistics, purportedly involved in the pathogenesis of this syndrome. Based on a mouse model, clindamycin would seem to be the antibiotic of choice.

SummaryNeoplasia and endocrine disorders are common complications of neurofibromatosis 1 (NF1-von Recklinghausen disease). We report here a case of NF1 in a 31-year-old woman presenting simultaneously with an abdominal neurofibrosarcoma and a pheochromocytoma. The prevalence of secondary neoplasia and endocrine tumors in NF1 is also discussed.

SummaryWe report the history of a 65-year-old woman, diabetic for 25 years who presented progressive alteration of consciousness with hyperthermia, right upper quadrant pain and vomiting.Bacteriological and radiographic investigations, especially computerized tomography scan of the abdomen, led to the diagnosis of emphysematous pyelonephritis caused by Escherichia coli.Despite a large spectrum antibiotic therapy and surgical drainage of the gas collection, the patient remained septic. An emergency nephrectomy was eventually performed.We review the characteristics of emphysematous pyelonephritis. Early diagnosis is essential because without early nephrectomy mortality rates reach 75%.

SummaryWe report the case of a 38-year-old patient with primary hyperaldosteronism. The diagnosis was made by the demonstration of a non-suppressible high aldosterone level in association with a hypokalemia, an inappropriate kaliuresis and low plasma renin activity. As the choice of the therapeutic approach is dictated by the subtype, further investigation was needed. Using a number of hormonal studies and noninvasive imaging techniques, we could establish the diagnosis of adrenocortical adenoma. Histological examination confirmed our diagnosis. We further discuss briefly the characteristics of the four subtypes of primary hyperaldosteronism and show that the used biochemical markers and imaging techniques are able to differentiate them.

SummaryA case of palmar fasciitis and arthritis (PFA) is described in a man with both a prolactinoma and metastatic cancer of the prostate. This rare condition is mainly described in women with ovarian cancer and our case is the first reported association of PFA with cancer of the prostate.
