SummaryThein vitroactivity of ampicillin, amoxicillin/clavulanate,Cefadroxil, cefaclor, cefuroximeCaxetil), co-trimoxazole, doxycycline, ciprofloxacin, ofloxacin, erythromycin, and roxithromycin was tested against unselected isolates ofS. pneumoniae(70),H. influenzae(93), andM. catarrhalis(46), cultured from clinically significant sputum samples of general Practice patients. All isolates of S. pneumonia were highly susceptible to ampicillin;Cefadroxiland cefaclor were markedly less active on a height basis; resistance was only observed with co-trimoxazole (4.3%), doxycycline (5.7%), and erythromycin (2.9%); however, ciprofloxacin and ofloxacin showed median MICs (MIC50), that were only one dilution below breakpoint,ß-lactamase was detected in 14.0% ofH. influenzaefolates; all isolates weresusceptibleto amoxicillin/clavulanate, cefaclor, and cefuroxime (axetil), although MICs were generally higher or cefaclor; the highest activity was exhibited by ciprofloxacin and ofloxacin; apart fromCefadroxil, erythromycin, and roxithromycin, that showed only marginal activity, resistance was observed with co-trimoxazole (4.3%) and doxycycline (1.1 %). All (including 71.7% ofß-lactamase producing) isolates ofM. catarrhaliswere susceptible to amoxicillin/clavulanate, cefaclor and cefuroxime (axetil), although MICs were markedly lower for amoxicillin/clavulanate; ciprofloxacin and ofloxacin showed the lowest MICs; resistance was only observed with Cefadroxil (2.2%). In conclusion, the antimicrobial agents showing the most uniformely highin vitroactivity against the 3 common community respiratory pathogens tested in the present study, were amoxicillin/clavulanate and, to a lesser extent, cefuroxime (axetil).

SummarySix patients with collagenous colitis are described and the literature is reviewed. Collagenous colitis is not a rare disease. Its most prominent feature is profuse watery diarrhoea. Although the endoscopic picture of the mucosa is usually normal, there is a microscopic thickening of the (subepithelial) basement membrane. For diagnostic purposes, multiple colon biopsies are warranted in patients with chronic or relapsing diarrhoea. There is no established therapy for this disease.

SummaryThe present study compares two new factor VIII preparations currently used in the treatment of haemophilia A. Factor VIII was partially purified from plasma obtained from unpaid voluntary Belgian donors in the blood transfusion centers of Lille and Amsterdam and virus-inactivated by exposure to solvent-detergent (FVI1I-SD) or by pasteurization (FVIII-P) respectively.The factor VIII content and the purity of both preparations were assessed in vitro, wheras in vivo we studied the recovery and the plasma half-life of both concentrates. The higher purity of FVIII-SD was confirmed. Factor VIII-P preparations contained more factor VIII than mentioned on the label. Both preparations gave good in vivo recoveries and half-lives. Patients who did not have antibodies to hepatitis B, hepatitis C or HI V at the initial screening, remained negative after six months treatment with the new concentrates. No patients developed neutralizing factor VIII antibodies. Furthermore patients appreciated the ease of administration of both preparations. In conclusion both FVIII concentrates are suited for the treatment of hamophelia A patients.

SummaryAntimitochondrial antibodies are found in 85 to 95% of patients with primary biliary cirrhosis. Nine different patterns have been identified but only four are associated with primary biliary cirrhosis. Anti-M2 antibodies are specific for the disease. The M2 antigen was found to be composed of five antigen determinants and related to the E2 component of three multienzyme complexes located within mitochondria. Anti-M4 and M8 antibodies appear invariably associated with anti-M2 and are markers for the«overlap syndrome»between primary biliary cirrhosis and chronic active hepatitis as well as for Poor prognosis. Anti-M9 antibodies are Preferentially associated with early and/or asymptomatic disease. Antinuclear antibodies are found in 24 to 58 % of patients with primary biliary cirrhosis. Six various patterns have been reported. Antibodies directed to the 200 kD polypeptide of>the nuclear pore and giving a perinuclear fluorescence are specific for primary biliary cirrhosis. Patients with such antibodies exhibit a less symptomatic disease and lower titers of anti-M2 than those without.

SummaryWe present the first case of visceral leishmaniasis (VL) in a Spanish patient with HIV infection living in Belgium. After four weeks of stibogluconate and zidovudine treatment, the initially low CD4 count improved, and the splenomegaly regressed. VL is becoming frequently reported in association with HIV infection, especially in countries where leishmaniasis is endemic. The apparent effect of VL on the CD4 count may cause problems in the staging of HIV infections.

SummaryTwo patients who developed a severe but; reversible cholestatic jaundice during and after treatment with an amoxicillin/clavulanic acid Preparation (Augmentin®) are described. The diagnosis of drug-induced intrahepatic cholestasis was based on the clinical course, biochemical pattern and on histological findings. Since amoxicillin alone does not induce liver injury, the combination of amoxicillin and clavulanic acid or clavulanic acid alone could be responsible for this adverse reaction, Probably through a metabolic idiosyncrasy or a hypersensitivity mechanism. Although more than 1 OOcases of hepatotoxic reactions have been reported to the pharmaceutical company (Beecham Laboratories) and several reports have been Published, the incidence of hepatotoxicity of this antibiotic is relatively rare, since this antibiotic has been widely used. However, recognition of this benign and reversible cholestatic syndrome is important to avoid unnecessary, expensive and invasive diagnostic procedures.

SummaryPorphyria cutanea tarda is a disorder of porphyrin metabolism that results from a deficiency of uroporphyrinogen decarboxylase, resulting in a characteristic pattern of porphyrin excretion. Elevated serum iron values are frequently observed among patients with porphyria cutanea tarda. The hypothesis has been advanced that a hemochromatosis allele is implicated in the clinical manifestation of porphyria cutanea tarda. We report the case of a patient suffering from both idiopathic hemochromatosis and porphyria cutanea tarda. The data of the medical literature concerning such an association are discussed.
