SummaryGallium scintigraphy was performed in 54 consecutive patients hospitalized for exploration of fever or inflammation of unknown origin, in whom thorough initial exploration did not yield any clue.Only in 28 patients (51.8%) could a definite diagnosis he established. The positive Gallium scintigraphy results were clinically rated as valuable, questionable or valueless according to their contribution to the diagnosis. Gallium scintigraphy was positive in 22 patients (40 %) and deemed valuable in 13 patients (24%).A final diagnosis could be ascertained in 77 % of the cases with a positive scintigram. In contrast a cause of fever or inflammation was found only in 34 % of the cases with a negative scintigram.Gallium scintigraphy is valuable as an early localizing investigation rather than a last resort test in patients with fever or inflammation of unknown origin. A positive scintigram increases the chances of establishing a final diagnosis.

SummaryRecently, new bcta-lactam antibiotics, such as imipenem/cilastatin (IMP) with an unusually broad antibacterial spectrum and especially an adequateP. aeruginosaactivity, have introduced the possibility of using prospective agent as empiric management of febrile granulocytopenic patients.We randomized 83 febrile neutropenic cancer patients for a prospective evaluation of two regimens: IMP versus piperacillin plus amikacin (PA). Both patients groups were similarly distributed with regard to age, sex, primary diagnosis and degree of granulocytopenia. More than 20% of the 74 evaluable patients had bacteraemia.The overall response rate for clinically or rni-crobiologically documented infections was 90 % in the IMP regimen versus 76 % in the PA regimen, but statistical difference was not achieved. All bacteraemias in the IMP group but only 60 % in the PA group were cured, however statistical difference was not achieved. IMP had to be discontinued in only one patient and the most common side effects were nausea and vomiting; no seizures were noted. Nephro-and ototoxicity, skin rash and bleeding have been the major side effects requiring drug discontinuation in 6 patients treated by PA.In conclusion, these data suggest that IMP used alone is safe and as effective as the combination of P plus A for the management of febrile granulocytopenic patients with haematologic malignancies. Further studies on a larger number of patients are needed to confirm these findings.

SummaryTwo cases of carcinosarcoma of the lung are presented. Until now, 76 cases have been recorded in the literature. The lesion contains an epithelial and a mesenchymal component, both with cytologic features of malignancy. A central endobronchial and a peripheral invasive type have been described. Fiberoptic biopsies usually show no tumour at all or contain only one of the two components. Treatment consists of lobectomy or pneumonectomy. Prognosis is not better for this tumour than for non-small cell bronchogenic carcinoma, and may even be worse.

SummaryWe report the case of a 25-ycar-old woman who voluntary swallowed mercury oxycyanide and presented an anuric acute renal failure. The treatment was classically based upon the use of chelating agents and haemodialysis. Recovery of renal function was excellent. We discuss then more widely the clinical, physiopathologic and therapeutic aspects of acute intoxication with inorganic mercury salts.

SummaryWc report a case of aneurysm of the abdominal aorta with spontaneous rupture in a 64-year-old man with emphysema and chronic alcoholism.Campylobacter fetussubspeciesfetuswas repeatedly recovered from the blood and from specimens of the excised aneurysm. The patient died in spite of early diagnosis, intravenous erythromycin and surgical intervention. The bacteriology and epidemiology of cam-pylobacteriosis in man is briefly discussed and eight other published cases of aneurysm, infected with the same organism, are tabulated.C. fetusssp.fetusshould be added to the list of bacteria having a tropism for vascular endothelium.

SummaryIn patients poisoned with a cholinesterase inhibitor, the diagnosis may initially be missed especially when no history is available from the patient or his relatives. The predominant respiratory symptoms may suggest pulmonary oedema as illustrated by the two case reports presented here. A high degree of suspicion and a careful clinical examination are necessary to make the correct diagnosis, which eventually can be confirmed by measurement of red cell and serum cholinesterase activity.
