SummaryThis retrospective survey describes the clinical, laboratory, radiological and pathological features of 11 patients with well-documented Wegener’s granulomatosis. Our findings are similar to those previously reported in large series. Despite treatment with cyclophosphamide and corticosteroids, 6 patients have died. It is suggested that the outcome in some of them would have been better if an appropriate therapy had been initiated earlier in the course of the disease. Likewise the renal outcome was primarily determined by the interval between the onset of the disease and the institution of treatment. Some diagnostic problems are discussed in this regard, e.g. the concept of limited forms of Wegener’s granulomatosis, the dissociation in time of the cardinal features and the lack of specificity of the radiological and laboratory data.

SummaryImmune reactivity was assessed by measuring skin reactions to 5 antigens and one mitogen on three consecutive days in a control population of 67 subjects and in patients with 12 different disease conditions.Highly significant differences between patient groups and controls in the means of the largest diameters or the frequency of positive skin tests were observed after 24 h. and, to a lesser extent, after 48 h. Abnormal skin reactions to tuberculin and trichophytin were only observed in a few patient groups, but when they did occur, differences from controls were highly significant (increased mean diameter in tuberculosis, total absence of tuberculin reactivity in Hodgkin disease, decreased trichophytin reactivity in sarcoidosis).On the basis of our observations, practical criteria for evaluating delayed cutaneous hypersensitivity can be proposed, and the value of standardized DCH in evaluating cellular immunityin vivoshould be reassessed.

SummaryThe management of severe life-threatening hypercalcemiain malignancy should start with intravenous fluid replacement; frusemide can be associated under scrupulous monitoring of fluid balance and electrolytes. Mithramycin is an efficacious adjuvant therapy, provided it is possible to wait 24 to 36 hours for mithramycin to become effective. Parenteral diphosphonates may prove to be useful in the control of malignant hypercalcemia, even in lifethreatening hypercalcemia.Calcitonin or corticosteroids on their own cannot be recommanded, though the combination deserves further assessment. Dialysis should be reserved for cases with marked renal impairment.Inmoderate hypercalcemiaonly two oral drugs are likely to be of significant help, corticosteroids or phosphates. The latter are more effective, but less acceptable to the patient. Increased fluid intake is an essential part of therapy. The next best therapy at present is intermittent, intravenous mithramycin at approximately one-week intervals. Some diphosphonates can be given orally, although the gastrointestinal absorption is variable.

SummaryIn this paper, the 4 major pathogenetic mechanisms of acute renal failure, arc critically reviewed. It is highly probable that in the ischemic models, both tubular obstruction and tubular back leakage play a dominant role; on the other hand, in the nephrotoxic models, hemodynamic intrarenal changes as preglomerular vasoconstriction and/or changes in Kfare more important, especially in the early initiation phase of these models. The tubular lesions occurring in established ARF, lead to failure of urinary concentration and decrease in tubular sodium reabsorption and are reflected in changes in urinary composition.The urinary parameters, especially fractional excretion of sodium (FENa) and renal failure index (RFI) can be used in the differential diagnosis between prerenal and renal failure. However, in clinical ARF where tubular obstruction plays a major role, both the FENaand RFI can be low.

SummaryA 35-year-old Rwandese patient presented with a dramatic multisystem disease, involving heart, lungs and liver. The final diagnosis of disseminated abscedating amebiasis was suggested by the clinical picture combined to the echographic presentation of the diseased organs revealing multiple cavities: the diagnosis was confirmed by echographically guided aspiration of the cavity contents. The combined involvement-of liver, lungs and pericardium by amebic abscesses occurs rarely and may be confused with other diseases where cardiac and pulmonary lesions coexist. Echography may help reach the correct diagnosis, by guiding the puncture of amebic fluid collections.

SummaryWe report the case of a young woman who presented an acute pancreatitis associated with a pneumonitis. Specific anti-mycoplasma complement fixing antibody titer rose sufficiently to be diagnostic of aMycoplasma pneumoniaeinfection. No other cause of pancreatitis was found. The possibility of an infectious aetiology and the physiopathology of this pancreatitis are discussed.
