SummaryThoracoscopywasperformedin 76 patients with chronic pleural effusions in which thoracocentesis and blind needle biopsy failed to reach a precise diagnosis.Analysisof thoracoscopic biopsiesprovided adefinitive diagnosis in 53 of the 76 subjects. The diagnoses’“eluded 35malignantdiseases in which the macroscopic appearance was diagnostic inonly27 Patients. Among these 76 patients, a talc poudrage was Performed in the management of recurrent effusion in 33 patients. No recurrence of pleural effusion was observed after 1 month in 87% and after 6 months in 76% of patients with recurrent pleural effusion. We confirm that thoracoscopy is a safe and useful technique forthe diagnosis of pleural effusion which substantially decreasesthe necessity for diagnostic thoracotomy and thattalc poudrage is effective to achieve pleurodesis.

SummaryEstrogen treatment in high doses is effective in reducing adult stature in constitutionally tall girls. In‘his study, growth data of 38 normal girls with a Predicted final height beyond 178 cm, are reported. They were treated with ethinyloestradiol in a daily dose of 0,200 mg until the epiphyseal plates were Practically fused. In addition, medroxyprogesterone acetate at a dose of 10 mg daily was given for 5 to 12 days every month. The reduction of final height occurred with decreased growth velocity and accelerated epiphyseal closure. The major factor affecting the response to treatment was skeletal age. The lower the skeletal age, the greater the difference between the predicted final height and the final height. This gain was directly related to the growth potential at start of therapy. The best time to start treatment night be the premenarche period at a bone age of 12 years. Serious side-effects were not reported, but treatment should be employed only when height Prediction is excessive.

SummaryDespite the use of increasingly potent antibiotics and aggressive cardiovascular monitoring and support, Gram-negative bacteremia and ensuing sepsis and septic shock remain a leading cause of morbidity and mortality after surgery and in critically ill patients.In previous years several new agents and techniques have been developed to improve management and outcome of severe Gram-negative infections. A recently introduced treatment is passive immunotherapy by administration of poly-or monoclonal antiendotoxin antibodies.The current view-sustained by experimental and human studies-on the mechanism of protection afforded by immunotherapy is that the harmful effects of endotoxin are neutralized by cross-reactive antibodies to the core glycolipid structure of rough mutant Gram-negative bacilli.Two recent large clinical trials reported impressive results achieved through the use of monoclonal antiendotoxin antibodies in certain subgroups of patients with Gram-negative sepsis. However, this treatment is empirical, expensive and it does not affect overall sepsis mortality.Cytokines such as tumor necrosis factor alpha and interleukin-1 play api votal role in sepsis. Experimental studies suggest that specific antagonism of the mediators might offer great perspectives for the treatment of Gram-negativesepsis.An early multi-pharmacological approach aimed a interruption of multiple steps underlying the inflaflamatory septic cascade will probably constitute the most promising future treatment of severe Gramnegative infectious disease.

SummaryA congenital C5 deficiency was the cause of recurrent meningococcal disease in a 27-year-old man.The lack of serum bactericidal activity accounts for the higher incidence and the different course of Neisseria infections in patients with a deficiency of the late complement components.Early antibiotic treatment and vaccination with a capsular polysaccharide meningococcal vaccine (A, C, Y, W-135) should be considered.

SummaryA 22-year-old woman developed transient left-sided ischemic colitis with submucosal oedema and bleeding, six weeks after an uneventful right hemicolectomy forCrohn’sdisease.Thethrombogenic properties of the contraceptive pill and the concomitant use of an ergotamine alkaloid were thought to be the cause of this complication in a patient at risk. An increase of procoagulant activity and underlying vascular injury has been described in Crohn’s disease.
