SummaryDothienenteric fevers treated with chloramphenicol show a marked shortening of their pyrexial period, generally associated with the disappearance of the amphibolic stage. Serious complications are less often observed.The mortality is not much influenced by the treatment which is usually undertaken too late in the course of the disease.Complications of therapy itself are negligible.

SummaryFrom October 4 to November 4, 1957, 16 patients were admitted into our department for pulmonary complications of influenza. Five of these individuals died. The group who recovered included 6 males and 6 females, from 16 to 62 year old.In 4 of those who recovered, the pulmonary complication started at the beginning of the disease. There was an interval of 3 to 7 days in seven. Seven cases were suffering from some predisposing condition such as asthma, mitral stenosis, chronic bronchitis, furunculosis or pregnancy.The series includes 10 cases of pneumonia or bronchopneumonia and 1 case of pulmonary abscess. Four cases of bronchopneumonia were respectively complicated by septicemia, pyothorax, pyopneumothorax or localized peritonitis. Specific antibodies were demonstrated in the serum in every case, but the virus could not be isolated from pharyngeal secretions in any case. Sputum cultures showed various pathogens which were sensitive to all common antibiotics, and in some treated cases, Coli Bacillus and Monilia. In the 4 cases with complications and in the one with pulmonary abscess, Staphylococcus Aureus was abundant, and sensitive to all antibiotics, except to penicillin.In the cases who died (5 females 16 to 51 year old) the complications followed the influenza without free interval. Two of these patients had no known previous disease. In three there were previous histories of epilepsy and diabetes, asthma and pregnancy, operated mitral stenosis and pregnancy. Four cases showed bilateral broncho-pneumonia on admission and one lobar pneumonia. Influenza virus was found in four cases and not looked for in one. In the five cases, lung culture obtained by lung aspiration showed Staphylococcus Aureus, sensitive to all antibodies, except generally to penicillin and streptomycin. Death supervened from 2 to 44 hours after admission, from asphyxia and shock, despite the administration of high dosages of antibiotics, oxygen, norepinephrine and hydrocortisone.The pathological findings included :a) Haemorrhagic pneumonia with suppurative bronchopneumonia;b) Epithelial necrobiosis followed by extensive desquamation of the bronchial mucosa.

SummaryA case showing clinical and radiological evidence of osteomalacia associated with pyelonephritis is reported. Right nephrectomy was performed because there was pyonephrosis on this side.Surgical procedure was followed by complete cure of the osteopathy. Metabolic data (table VII) indicate hypophosphatemia, hypomagnesiemia and excessive urinary loss of P, Mg and Ca before operation.Although a slight systemic acidosis was present, no abnormality of renal acid producing mechanisms was elicited.After operation renal handling of P, Ca and Mg was shown to be essentially normal.These facts demonstrate that, in this case, the renal tubular defect responsible for metabolic disturbances was limited to the right kidney.

摘要:
SummaryA case of plasma-cells thyroiditis is described. The plasma-cells infitration precludes its classification among the usual chronic thyroiditides.According to the work of Doniach and Witebsky the pathogenic basis of this thyroiditis could be an auto-immunological process. Serologic tests performed in this case have proved this assumption.The necessity of early therapy with corticoids is stressed with the hope of inhibiting the«chain reaction»leading to the destruction of the gland.

SummaryMyocardial involvement is frequent during typhoid fever and is not paralled by the severity of the primary disease. Myocarditis may appear at the prodromal, acute or convalescent stage of typhoid fever. The onset of this complications is quite variable : it can be insidious or outburst suddenly with vascular collapse and angina pectoris. In serious cases congestive heart failure supervenes. Recovery is usual.The pathologic aspect of the heart is the same as in other infectious diseases. Coronary thrombosis has been described as a rare complication of typhoid fever in two cases.The electrocardiographic changes of the myocarditis in typhoid fever are :a) non specific alterations of the ST segment and T wave with diminution of amplitude of the R wave;b) lesions of the conductive system, such as increased PR or QT intervals and slaggering of R wave.
