SummaryAmidolytic assays for Factor X with the chromogenic substrate S-2337 were found to be accurate and reproducible. Reference values in a normal population were 8.7–17.4μkat 1-1with a mean values of 13.1±2.2μkat 1-1; the distribution is normal with a statistically significant difference between males and females.A reasonable correlation was obtained between prothrombin time values and amidolytic Factor X determinations in 44 patients on oral anticoagulant therapy (correlation coefficient 0.73). Fully concordant results were obtained in 73 percent of the samples whereas fully discordant results were not observed. Prothrombin times between 15 and 25 % of normal (ratio between 1.7 and 1.3) corresponded to Factor Xa levels between 3.5 and 5.1μkat 1-1(27 to 39 percent of the reference value). Fully concordant results were obtained in 88 percent of the samples of patients treated with fenprocoumon (n = 34) but only in 40 percent of the patients treated with acenocoumarol (n = 10).Factor X measurements were found to be equally sensitive as prothrombin time assays to small decreases of vitamin K-dependent factors in patients with liver disease.

SummaryClinical and serological data of 5 patients with amoebic liver abscess are described. They were infected during a stay in Africa or Asia. Follow up shows that the cavities after successful medical therapy only decrease with a rate of 0.07–0.12 mm per day. It can thus take years before a large abscess disappears completely.

SummaryWe present five patients (mean age±1 SD: 48±19 yrs) with cardiac sarcoidosis.Four patients had conduction disturbances, three showed important arrhytmias on electrocardiogram and one presented heart failure. Holter monitoring and/or stress testing was abnormal in all subjects studied. The201thalliumscintigraphy performed in three patients demonstrated perfusion defects compatible with myocardial sarcoidosis.Before the institution of corticosteroid therapy, several patients received anti-arrhythmic agents, which, however, were not effective. In two patients a pacemaker was implanted. One patient, who refused a pacemaker, recently died.

SummaryDuring sporting events that took place under weather conditions unusual for our country, the authors observed 5 cases of exertional heat exhaustion and 4 cases of exertional heat stroke, one of which was fatal. The clinical pictures and the main biological values are summarized. A reminder of the physiology of body temperature regulation in normal man is followed by a review of the literature about heat stress disease. This pathology, altough exceptional in our country, may affect vital organs and lead to dramatic consequences if not recognized and treated early. Simple preventive measures nevertheless remain the best treatment.

SummaryIntrathoracic goitre is one of the rare, benign causes of potentially curable superior vena cava syndrome (SVCS). We report two cases of colloid in trathoracic goitre responsible for SVCS, as confirmed by simultaneous thyroid scanning and isotopic superior cavography. Both patients underwent surgical excision of their goitre and had a complete cure of their superior vena cava syndrome. This report illustrates the importance of considering be nign etiologies of superior vena cava syndrome be fore starting emergency radiotherapy.

SummaryA 46-year-old woman with an adrenal pseudocyst and arterial hypertension is presented. Intracystal hemorrhage led to an operation which confirmed the cystic nature of the lesion. Contrary to some previous experiences, the hypertension remained unchanged postoperatively.This report illustrates the usefulness of ultrasonography in this condition. The association of adrenal cyst and hypertension is reviewed.

SummaryImportant losses occur during the intravenous infusion of the organic nitrates glyceryl trinitrate and isosorbidc dinitrate, if appropriate precautions arc not taken. The literature data discussed here show that the loss is mainly important when PVCcontaincrs orinfusion sets are used. It is recommended to add the nitrates to the infusion fluid in glass containers, immediately before use. For the infusion set, high density polyethylene tubing is suitable; if PVC-tubing is used, pre-equilibration should be performed. If knowledge of the exact amount administered is important, infusion by means of a glass or propylene seringue, coupled to polyethylene tubing, is to be preferred.
