Summary1. A study of 81 electrophoretic patterns of serum of diabetic patients shows a high frequency of protein disturbances, of which the most constant are a decrease in the amount of albumins and an increase in the amount of and especiallyftglobulins.2. In the uncomplicated diabetic and especially when the disease is benign, as in obese adults, the protein disturbances are relatively slight. In the forms more serious there is a clear increase of the electrophoretic anomalies : selective increase of the a2globulins in the juvenile diabetes; a marked decrease of the albumins and a considerable increase of theftglobulins in diabetic adults with severe weight loss.3. The protein alterations exist already in the course of the first months or years of the disease. If the disease is not complicated by degenerative lesions, only the hypoalbuminemia seems to increase in the course of the evolution of the diabetes.4. The age of the patient intervenes in these protein modifications in predisposing adult subjects to a decrease of the albumins and to an increase of theftglobulins.5. The appearance of vascular complications exaggerates simultaneously the decrease of the albumins and the increase of the amount of a2andßglobulins. The Kimmelstiel-Wilson syndrome is especially characterized by a decrease in the albumins and a considerable increase in a2globulins.6. Infections and pregnancy modify the electrophoretic picture in the same manner as in subjects non-diabetic.7. In the case of severe hemochromatosis the electrophoretic pattern shows a normal or even a decreased amount offtglobulins.8. At this time the interpretation of these anomalies can only be hypothetical. The hypoalbuminemia seems to result either, from a trouble in synthesis by the liver, or from an exaggerated protein catabolism. The hyperglobulinemia a2could be interpretated, in certain special pathological conditions (hyalinosis) to an augmentation of the level of serum polysaccharides. The increase of theftglobulins signifies probably an enrichment of the serum in lipoproteins of high molecular weight to which one attributes an«atherogenic»role.

SummaryThe 3 - (1’- phenylpropyl) 4 - hydroxycoumarine (marcoumar) is an efficacious anticoagulant; because of its long and definite action, it is particularly maniable.Its action is characterized by a drop of Koller factor VII and prothrombine and can be controlled by Quick method or more easily by the Marbet and Winderstein method.During the treatment of acute thrombosis, simultaneous heparin administration is necessary until the full action of the drug is reached.36 patients have been submitted to the action of this drug. All the cases of spontaneous or postoperative venous thrombosis responded very well. No case of pulmonary emboli has been observed during the evolutionary course of venous thrombosis, and no recidive in patients having presented previously pulmonary emboli.Hemorragic accidents consisted in microscopic hematuria (8 times out of 36) and deep hematoma (one out of 36).Synthetic vit. K1emulsion is an efficacious antidote bringing back quickly the prothrombine time to harmless levels.

SummaryThe authors recall that gastric neoplasm is very frequent, almost inoperable, and that the after results are desastrous. From the study of 150 cases, they tried to deduce the tactical leading principles to improve the after results of gastrectomy for gastric neoplasm. It seems that female is frequently affected because she represents 30°Qof the total of cancers. The nosological study shows that every man after forty who«feels that he has got a stomach»must be suspected of evoluting neoplasm. The most dangerous cases are those which present the ulcer mask, of which the pains are relieved by feeding. The niche even may disappear at the XRay by filling or scarring of the peptic ulceration of the neoplastic lesion. The same cases otherwise are normal or hyper-chlorydric and that means that gastric intubation helps very little in diagnosis. By examining the future of gastric polypous, the authors reach the conclusion that all become malignant with time and must be extirpated. From rigorous histological criteria, they estimate malignant transformation of the ulcer certain in 10°/aof the cases at least. For the authors, all gastric lesions appearing after forty must be taken in charge by the surgeon : to wait for the clinical confirmation of the cancer means to condemn the patient.

SummaryThe actual methods of investigation of the cardio-pulmonary function and especially the cardiac catheterization, have allowed to realize great advances in the study of the pulmonary circulation. Accurate notions have been obtained concerning gaseous exchanges, blood flow through the lung and pulmonaries pressures; however the problem of the intra-thoracic blood volume remains vet much discussed.The pressures of the pulmonary circulation are very different from those of the systemic circulation : the arterial pressure is lower and the venous pressure is higher in the first than in the second case; this depends and the anatomical and physiological characteristics of the pulmonary circulation and of the left atrio-venous system.The pressure in the pulmonary artery may be increased secondarily to an augmentation of the pressure in the left auricle, or may increase after a change of the arteriolar resistance. The latter appears either for a considerable increase of the blood flow or for a reduction of the capacity of the vascular bed. This reduction is due to anatomical lesions or to a vasoconstriction caused by anoxemia, hypercapnia, intervention of the nervous system or to the action of some drugs.The vasoconstriction of the pulmonary vascular bed seems very likely, but the importance of the factors considered and their mechanism remain discussed.If we consider the different clinical conditions that are accompanied with pulmonary arterial hypertension, we see that for some of them, the mechanical factor (left auricular hypertension) is preponderant, though the modifications of the blood volume, of the cardiac flow and of the arteriolar tonus are also important; this is the case in left ventricular insufficiency, mitral stenosis and constrictive pericarditis. In chronic lung diseases, the part played by the functional factors is probably much more important, but must not be exaggerated; especially in pulmonary emphysema, the respective parts played in the development of the hypertension, by the anatomical lesions and by the physiopathological troubles consecutive to anoxemia, still remain discussed. Finally, in case of«primary»pulmonary arterial hypertension, isolated or associated to congenital cardiovascular malformations, it is perhaps a residuum of the foetal physiology, but the exact mechanism of the increase of the arterial resistance still remains to be precised, as well as the histological characteristics of this affection.At the present time it is possible to establish a nosological list of the types of pulmonary arterial hypertension based on the causal disease or on the more important pathological mechanism. (List 7.)But we must recognize that, in each case, the factors are numerous and that the part played by each of them is difficult to precise; these mechanisms are often associated, the existence of some among them remains discussed and the way of action of others is not yet determined.
