SummaryThe survey of 35 cases of primary chronic adrenocortical insufficiency (Addison’s disease) in the adult, led to a reappraisal of the main features of this condition and to emphasize important practical aspects of its treatment. The aetiology was of the auto-immune type in the majority of cases, with frequent additional endocrine disorders that developed before as well as alter the onset of adrenocortical failure. For this reason alone, a systematic follow-up of patients with Addison’s disease is a must.

Summary25 young asymptomatic heroin addicts were investigated. Pulmonary perfusion scintigraphy was altered in 94. The functional testing of pulmonary function performed in 9 of them revealed a diminished diffusion capacity in 2. Liver function tests were slightly altered in 40 %. Hepatic scintigraphy showed functional heterogeneity in 87 % with a splenic hyperactivity in 47 %. AgHBs was positive in 1 out of 24 cases tested. These data are compared with those reported in the U.S.A. and their pathophysiology is discussed.

SummaryTwo adult cases with the hemolytic uremic syndrome are reported. The first patient died and this illustrates the often bad prognosis of this disease in the adult. The simultaneous occurrence of the familial and recurrent variant of this syndrome in the second patient is highly remarkable. The differential diagnosis with for example thrombotic thrombocytopenic purpura is discussed. A high antibody titer against Coxsackie B, virus was measured in the serum of our first patient, while a rise in antibody titer for mononucleosis infectiosa was observed in the second patient. The role of viral infections, of intravascular coagulation and of immunological phenomena in the pathogenesis of the hemolytic uremic syndrome and the therapy with heparin and platelet inhibitors arc discussed.

SummaryThe cutaneous reactivity to tuberculin and DNCB, the in vitro lvmphocyte transformation to phytohaemagglutinin, and serum immunoglobulin concentrations were studied in 36 sarcoid patients and 50 controls.The data show a decreased skin reactivity to the antigens us:d. When compared to an age matched control group, there was only a significant difference for the reactivity to PPD. Tuberculin anergy was most pronounced in patients with pulmonary infiltrates and hilar lymphadenopathy (clinical stage II) suggesting that cutaneous reactivity is more depressed in patients with a more active stage of the disease. In vitro lymphocyte transformation was decreased in sarcoid patients but this was not statistically significant. An increase in IgA and IgG level was noted in sarcoid patients but was not statistically significant when compared to the age matched control.

SummaryTuberculous dactylitis is easily curable by specific chemotherapy but the diagnosis is often overlooked mainly because of the relalative rarity of skeletal tuberculosis, the deficiency of the initial X-ray findings and the absence of fast bacilli in the smears from sinus tracts.The recovery in these sites ofStaphylococcus aureusmerely signifies a colonising by these bacteria but discards the diagnosis outright, while a biopsy specimen of tissue will often assert it.
