SummaryWe evaluated the diagnostic usefulness of capnography curves during and following a hyperventilation provocation test (HVPT) in the hyperventilation syndrome (HVS). The diagnosis of HVS was based on the Nijmegen questionnaire and on the reproduction of symptoms during HVPT. Capnography curves of 40 HVS patients, 40 non-HVS patients with psycho-somatic complaints and 26 healthy controls were analyzed. There was no difference in baseline end-tidal CO2-level (FETCO2) between the 3 groups. The spontaneous fall of FETCO2during the adaption phase was clearly different in HVS patients versus non-HVS patients or controls: -0.12 mmmol/1 (95 % confidence limits-0.18 to-0.06) versus + 0.01 mmol/1 (95 % confidence limits-0.04 to + 0.16) (p = 0.002). The 3 minutes FETCO2recovery ratio and the 5 minutes ratio were not significantly different between the groups. In conclusion, in this study the spontaneous fall of FETCO2during the adaption phase of the HVPT was the only valuable part of the capnography test to discriminate between HVS and non-HVS patients.

SummaryThe effect on the clinical status of catabolic hemodialysis patients of I.V. essential amino-acids administered over 3 months at the end of each dialysis is assessed in an open clinical follow-up study of 10 patients; these patients showed a progressive deterioration of general condition and a progressive weight loss in the period before the start of the treatment. The study was undertaken in a hospital dialysis unit, with as main outcome measures body weight, hematocrit, a scoring index of general condition and degree of edema. In patients showing a progressive and consistent loss of body weight in the months preceding the study, after the first treatment month, body weight started to rise, increasing after 3 months from 56.2±2.3 to 58.6±2.4 kg (p<0.01). The hematocrit raised from 22.4±1.6 % up to 26.5±1.9 % (p<0.02).Over this period, only 2 liters of packed cells were administered, in contrast to an overall need of 13 liters in the preceding 6 months. Peripheral and/or pulmonary edema disappeared. A scoring index, of general condition, increased from 5.1±1.5 before the start of the study to 11.7±0.8 after 3 months (p<0.01).It is concluded that the parenteral administration of amino-acids in catabolic patients on chronic hemodialysis has a beneficial effect on general condition, and the balance of body fluid and body mass.

SummaryTwenty-six patients underwent a polysomnygraphic study allowing sleep staging and respiratory events scoring with the use of the oronasal flow, abdominal, thoracic and total displacement (Respitracet), and ear oxymetry. Moreover the patients were also equipped with a tracheal microphone giving a power rectified enveloppe (sonospirogram).Eleven patients showed abnormal respiratory events that were scored by visual lecture using all respiratory Parameters (excluding the sonospirogram) and were classified as obstructive central and mixed apneas-hypopneas. Periodic breathing was also appreciated. Detection of the same events was tried with the sonospirogram alone.The sonospirogram could accurately detect snoring and periodic breathing and finally central obstructive mixed apnea (the apneic index being well correlated: P<0.0()1 as well as the mean apnea duration: p<0.005).In contrast hypopneic events related to snoring could not be accurately appreciated.We conclude that a sonospirogram may be useful for the detection of abnormal respiratory events when used alone (screening) as when added to other respiratory signals.

SummaryWith 30 years of experience, it is possible to claim that diuretics are well tolerated antihypertensive drugs Educing a significant decrease in systolic and diastolic blood pressure as good as obtained with B-blockers, inverting enzyme inhibitors, calcium antagonists and centrally acting drugs.Recently diuretics have been criticised because of a substantial number of important side-effects, and are disregarded by some authors as first-line agents for the treatment of mild hypertension. Diuretics seem however safe in the majority of hypertensive patients who do not present special problems. Using low doses, which are equally effective, most toxic properties can probably be avoided. Combination therapy, especially with inhibitors, has not only additional effects, but lowers also the potential adverse effects, caused by Sureties. We are convinced that diuretics are still important as first-line agents for the treatment of Serial hypertension in the general population.

SummaryOn October 1989, eosinophilia and incapacitating myalgia, together with arthralgia, dyspnea, cough and edema of the extremities, were shown to be associated with L-tryptophan ingestion. Since then, 1531 cases of eosinophilia-myalgia syndrome have been reported in United States and 22 in Belgium.We report here the unusual pulmonary presentation of this syndrome with a dramatic response of eosinophilia to corticotherapy. The cardio-pulmonary symptoms of eosinophilia-myalgia syndrome and its pathophysiology, which remains unclear, are discussed.The withdrawal of the substance and corticotherapy generally lead to complete recovery although several deaths have been reported.
